This refers to the inspection conducted by Messrs. 1. Bernes and R. G. Taylor '

of this office during the period July 11 through August N 1988, af the

activities authorized by NRC Operating License DPR 34 for the Fort St. Vrain  :

'

Nuclear Generating Station, and to discussion of our findings with Mr.

R. O. Williams and other members of your staff at the conclusion of the i

, inspection.

Areas examined during the inspection included licensee event report followup,

control of design changes and inodifications, and your activities related to

refurbishment of the reactor helium circulators. Within these areas. the

inspection consisted of selective examination of procedures and represei.;;ative -

records interviews with personnel, and observations by the NRC inspectors.

The inspection findings are documented in the enclosed inspection report.

During this inspection, it was found that certain of your activities were i'- t

i violation of NRC requirements. Consequently, you are required to respond to

'

these violations, in writing, in accordtnce with the provisions of ,

Section 2.201 of tne NRC's "Rules of Practice." Part 2. Title 10. Code of  ;

Federal Regulations. Your response should be based on the specifics contaired

in the Notice of Violation enclosed with this letter.

1 One unresolved item is identified in paragraph 5.d of the enclosed inspection

report.

Also during this inspection, it was found that certain of your activities

appear to deviate from comitments made to the NRC. This item and references

to the comitment is identified in the enclosed Notice of Deviation. You are

requested to respond to this deviation in writing. Your response should be e

based on the specifics contained in the Notice of Deviation enclosed with this  !

We have also examined actions you have taken with regard to previously

irtentified inspection findings. The status of these items is identified in

paragraph 2 of the enclosed report.

The inspection report (Section 2.b) notes that while progress has been made to

improve your corrective action program, management needs to direct renewed

emphasis toward further improvement in the program. Of particular concern is

'

the relatively large rumber of Nonconfonnance Reports from 1986 and earlier

that are unresolved for a variety of reasons. Further managen nt attention is

needed relative to the corrective action program.

The inspection report identifies apparent v ulations related to helium

l. circulator installation without appropriate authorizatien and failure to

) control special 3rocesses. While written response is required within 30 days

1 of the date of tiis letter, we also believe it is appropriate to meet with

you to obtain an earlier indication of how you plan to address the technical

1 aspects and generic implications related to the apparent violations. We will

j contact you separately to arrange a convenient time for a management meeting

,

which should be held prior to restart from the present outage.

The responses directed by this letter and the accorrpanying Notices are not

subject to the clearance procedures of the Office of h tagement and Budget as

required by the Paperwork Reduction Act of 1980. PL 96-511.
