SEQUOYAH NUCLEAR PLANT UNIT 2 SPECIAL REPORT 88-09 DESCRIPTION OF EVENT On Apell 15, 1988, with unit 2 in mode 5 (0 percent power, 4 psig, 124 degrees F), a fire barrier (biological chield blocks, Shield Building equipment hatch elevation 734) was nonfunctional for an interval greater than seven days.

On Apell 8, 1988, unit 2 was returned to cold shutdown, and biological shield blocks located in front of Shield Building equipment hatch on elevation 734 were removed to provide access in the Reactor Building for steam generators tube leak testing and repair activities.

An hourly fire watch was established as required by the action statement of Limiting Condition for Operation (LCO) 3.7.12.

The biological shield blocks provide a fire barrier and were not reinstalled within seven days, requiring a special report in accordance with Technical Specification (TS) LCO 3.7.12.

CAUSE OF EVENT The steam generator tube leak testing and repairs could not be completed in seven days, and it was decided to leave the equipment hatch open until completion of the steam generator tube testing and repair activities.

ANALYSIS OF EVENT This event is reported in accordance with the requirements of action statement (a) of LCO 3.7.12.

A roving fire watch was established to inspect the Shield Building egulpment hatch area on an hourly basis as required by the action statement of LCO 3.7.12.

The existing fire detection and fire suppression system for the affected area are operable and would actuate in the event of a fire.

Therefore, there is no danger to safety-related equipment.

CORRECTIVE ACTION A roving fire watch at a regular interval of one hour was established and was maintained until the biological shield blocks were reinstalled, and the fire barrier was declared functional on May 4, 1988.
