APPENDIX A NOTICE OF VIOLATION Rhode Island Atomic Energy Commission Docket No. 50-193 Nuclear Science Center License No. R-95 During NRC inspection from March 28-31, 1988, a violation of NRC requirements was identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, that violation is listed be-low:
Technical Specification K.3.e(4)(f)2 requires that the fission density of all fuel elements which have burnup shall be calculated at least quarterly.
Contrary to the above, the required fission density calculation was not com-pleted between 9/30/86 and 2/13/87, or between 10/27/87 and 2/24/88.
This is a Severity level V Violation (Supplement I).
Pursuant to 10 CFR 2.201, Rhode Island Atomic Energy Commission is required to submit to this office, within 30 days of the date of the letter transmitting this Notice, a written reply including: (1) the corrective steps which have been taken and the results achieved, (2) the corrective steps which will be taken to avoid further violations, and (3) the date when full compliance was achieved. Where good cause is shown, consideration will be given to extending the response time.
