Additional Information for Eastman Kodak License SNM-1513 Renewal To the NRC:

As per recent telephone conversations with Mr. Sean Soong of your office, Eastman Kodak is providing additional information and clarification to our radioactive material license application (license SNM-1513), originally submitted March 23,1998.

1.

Hydronen to Uranium (H:U) Ratio Clarification The correct H:U ratio is 426 to 1. The ratio stated on page 8 of the application (Section 4.3.3.2) is incorrect and will be changed.

2.

Health Physics Procedures Written procedures for carrying out health physics activities shall be established and maintained, and shall be reviewed every two years by the Radiation Safety Officer.

Section 6.4, #2 of the application ("Operaiing Limitations") on Page 16 is re-worded as follows:

" Samples will not be introduced into the neutron activation positions of the CFX without approval of the authorized operators. It will be the responsibility of the Group Leader to assure that no materials of unknown nature, no Special Nuclear Materials (SNM), and no materials which may be detonated are permitted to enter the sample j
