(NRC INSPECTION REPORT NOS. 50-321/88-21 AND 50-366/88-21)
This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by
G. R. Wiseman and D. C. Ward on June 20-24, 1988.
The inspection included a
review of activities authorized for your Hatch facility. At the conclusion of
the inspection, the findings were discussed with those members of your staff
identified in the enclosed Inspection Report.
Areas examined during the inspection are identified in the report.
Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
The inspection findings indicate that certain activities appeared to violate
NRC requirements.
The violation, references to pertinent requirements, and
elements to be included in your response are described in the enclosed Notice
of Violation.
The violation described in the enclosed Notice (related to documentation of

fire protection activities) is similar to a violation contained in the Notice
sent to you by our letter dated December 18, 1987. Because "similar violations,"
j
as described in the NRC Enforcement Policy, are of significant concern to the
'
NRC, please give particular attention in your response to the identification of
L
the root cause of this problem and your corrective action to prevent recurrence.
We also encourage more aggressive management attention to site Fire Brigade
j
performance.
We are concerned that the continuing deficiencies in the quality
,
of the Fire Brigade leadership performance during the fire drills witnessed
i
during this inspection and in a previous inspection (Report No. 87-30, dated
'
December 18,1987) indicate a weakness in Fire Brigade leadership training. A
fully effective training program and management followup would result in timely
l
correction of the above weakness.
In accordance with Section 2.790 of the NRC's "Rules of Practice," Part 2
,
l
Title 10, Code of Federal Regulations, a copy of this letter and its enclosures
will be placed in the NRC Public Document Room.
