Please be advised that your license expires at the end of the day, in the month, and year stated in the license. Unless your license has been terminated, you must conduct your program involving byproduct materials in accordance with the conditions of your NRC license, representations made in your license application, and NRC regulations. In particular, note that you must:

1. Operate Iri accordance with NRC reguutions 10 CFR Part 19, " Notices, Instructions and Reports to Workers; Inspections," 10 CFR Part 20, " Standards for Protection Against Radiation," and other applicable regulations.
2. Notify NRC, in writing, within 30 days:
a. When the Radiation Safety Officer permanently discontinues performance of -

duties under the license or has a name change; or

b. When the mailing address listed on the license changes. (No fee is required if the location of byproduct material remains the same.)
3. In accordance with 10 CFR 30.36(b) and/or license condition, notify NRC, promptly, in writing, and request termination of the license when a decision is made to terminate all activities involving materials authorized under the license.
4. Request and obtain a license amendment before you:
a. Change Radiation Safety Officers;
b. Order byproduct material in excess of the amount, or radionuclide, or form different than authorized on the license;
c. Add or change the areas of use or address or addresses of use identified in the license application or on the license; or 3Y

d. Change ownership of your organization.
5. Submit a complete renewal application with proper fee or termination request at least 30 days before the expiration date of your license. You will receive a reminder notice approximately 90 days before the expiration date. Possession of byproduct material after your license expires is a violation of NRC regulations. A license will not nonnally be renewed, except on a case-by-case basis, in instances where licensed material has never been possessed or used, in addition, please note that NRC Form 313 requires the applicant, by his/her signature, to verify that the applicant understands that all statements contained in the application are true and correct to the best of the applicant's knowledge. The signatory for the application should be the licensee or certifying official rather than a consultant.

You will be periodically inspected by NRC. Failure to conduct your program in accordance with NRC regulations, license conditions, and representations made in your license application and supplemental correspondence with NRC will result in enforcement action against you. This could include issuance of a notice of violation, or imposition of a civil penalty, or an order suspending, modifying or revoking your license as specified in the General Statement of Policy and Procedure for NRC Enforcement Actions. Since serious consequences to employees and the public can result from failure to comply with NRC requirements, prompt and vigorous enforcement action will be taken when dealing with licensees who do not achieve the necessary meticulous attention to detail and the high standard of compliance which NRC expects of its licensaes.

We wish to name John Idoine as the Radiation Safety Officer (RSO) and Timothy Sullivan as the

, Alternate RSO Timothy Sullivan will act as RSO only in the physical absence of John Idoine.

Both of these individuals are fully qualified to act as RSO. John Idoine served as the RSO on this license for ten years prior to November 1994. His qualifications are discussed in item #2 of a letter dated January 13,1984. Timothy Sullivan is the current RSO. His qualifications are presented in a letter dated August 3,1994.

Dear Ms. Holt:

This'1etter is in response to our telephone conversation of January 9, 1984.- It is an updated version of my letter of October 7, 1983 which was received and acknowledged by you, but has evidently been misplaced. This letter provides additional information supporting renewal of our NRC license '

This is an application for amendment of our license number SNM-562 to change the Radiation Protection Officer from Professor John Idoine to Professor Timothy Sullivan.

Professor Idoine will be on leave during the academic year '94 '95.

Attached is my resume. I have 20 years experience as an experimental physicist. I have two years experience in high energy physics and four in plasma physics where I worked with (accelerator) sources of X- and gamma radiation. In my three years of work at Los Alamos National Laboratory, I was trained in laser safety and the handling and minimization of chemical waste. For the last three years, I have taught Introductory Physics lab, which included performing one of the primary labs that involve the Pu-Be source. I have also taught Modern Physics for three years which includes a three week segment on nuclear physics. This summer, I successfully completed a forty hour course of training offered by CSI-Radiation Safety Training of Kensington, MD. The course was entitled " Radiation Safety Officer" and was conducted by Ray Johnson, C.H.P., P.E. The course covered all topics listed in NRC Regulatory Guide 10.3 section 4.4.

I should also be listed as the contact person for this license. I can be reached .by phone during working hours at (614) 427-5830, at home (614) 393-3510. My Internet email address is sullivan@kenyon.edu.

Thank you for your time and attention.

Dr. Paula Tumer Kenyon College 740-427-5367 suanCT License No.: SNM-562 Control No.: 304103 suna w n We have reviewed your letter dated June 15,1998, requesting an amendment to your NRC License No. SNM-562 and find that we will need additional information as follows:

Please be advised that a better way to structure your request in order to automatically substitute one RSO for the other would be to request that one individual (either Dr. Sullivan or Dr. Idoine) be named as RSO to the license and the other individual be named as the ALTERNATE RSO.

You must stipulate that THE ALTERNATE RSO WOULD ACT AS RSO ONLY IN THE PHYSICAL ABSENCE OF THE NAMED RSO.

Do not use the term ' assistant RSO' because it does not convey that each RSO is equivalently ,

trained and experienced to operate the radiation safety program. I Please orecare a written resnonse within 15 days. address it to my attention at the above address, be sure to call it " additional information to Control No. 304103" and we'll need a management signature on the letter to authorize the request. Dr. Sullivan signed the 6/15/98 ,

letter but, unless management designates an authorized signatory to us in writing, such as the j RSO or a mid-level manager, we must have senior management's concurrence on all correspondence via signature.

(No response needed item: please do not send us CVs or resumes in the future. They contain personal information that we then must protect but did not ask for and they frequently do not contain the types of information that we do need.)
