SUBJECT: BYRON INSPECTION REPORT 50-454/98017(DRP); 50-455/98017(DRP)

AND NOTICE OF VIOLATION

'l Dear Mr. Kingsley:

On August 24,1998, the NRC completed an inspection at the Byron 1 and 2 reactor facilities.

The enclosed report presents the results of that inspection.

During this inspection period, we noted that control room activities were consistently performed

in a professional manner. This was most evident by your response to the Unit i loss of offsite

power event. We observed effective oversight by operations department management and

strong command and control throughout the event recovery.

However, we are concemed with the trend in configuration control events. Since  !

January 1998, you have identified numerous examples of failure to maintain control of the

configuration of plant systems and equipment, most of which involved nonsafety-related

balance of plant systems and did not result in adverse safety consequences. While we

recognize that you are in the process of developing a comprehensive corrective action plan to

address the issue, we are concemed that to date, your corrective actions have not been

effective at arresting the trend of configuration control events.

One violation of NRC requirements was identified during this inspection period. This violation

involves the failure to maintain the main control room ventilation envelope configuration due to

inadequacies in your ventilation barrier impairment process. The violation is cited in the

enclosed Notice of Violation (Notice) and the circumstances surrounding it are described in

detailin the enclosed report. The NRC has concluded that information regarding the reason

for the violation,- the corrective actions taken and planned to correct the violation and prevent

recurrence is already adequately addressed on the docket in Licensee Event Report (LER) -t

50-454/98016, dated August 6,1998. Therefore, you are not required to respond to this letter

unless the description therein does not accurately reflect your corrective actions or your t

- position. In that case, or if you choose to provide additionalinformation, you should follow the

instructions specified in the enclosed Notice.
