- The storage container is a rectangular box with handles at each end and a. hinged top (Attachment 1).

It is 14-1/8" (35.88 cm) in length,-8M" (21.59 cm) tall and SM" (13. 97 cm) wide.

The bottom 5" (12.5 cm) of the container is filled with lead and has four compartments for storing two each of Cobalt-60 and Cesium-137

. sources.

The sources are contained in a right circular cylinder of depleted uranium approximately 2M" (6.35 cm) in diameter and

/2X"1(6. 99 cm) high-(Attachment 2).

The depleted uranium shield hasta cone shaped collimator cut at'one end'and is housed in a stainless steel can.

At the end opposite the collimator is a "T"

shaped; handle.

LABELING:

' Each. source holder is labeled as: radioactive material, VD (HP),

serial number,' source, activity and date.

The storage container L

is labeled with a stainless steel name plate that states,

OUALITY ASSURANCE AND CONTROL:

Not applicable.

LIMITATIONS AND/OR OTHER CONSIDERATIONS OF USE:

1.

Provisions for the 36 pounds (16.33 kg) of depleted uranium should be made on the license.

2.

The container and source holders must be returi.ed to the manufacturer for source replacement.

SAFETY EVALUATION:

The Model Intec Control Isotope Container device is no longer manufactured or distributed by Gamma Industries, Inc. and Gamma Industries, Inc. is no longer in business.

Therefore, the registration certificate is being converted to an inactive registration certificate.

The device was originally approved for licensing purposes by the Louisiana Division of-kadiation Control.

NRC attempted to obtain copies of all-information submitted by the registrant in support of the evaluation.

However,.NRC could not locate all the information.

Therefore, no safety evaluation of the device was performed as part of the conversion of the certificate to

-inactive status.
