Licensed material may be used only at the licensee's facilities located at the Department of Physics,2152 Hillside Road, University of Connecticut, Storrs, Connecticut or Department of Environmental Health and Safety, Hazardous Waste Facility, Horse Barn Hill Road, University of Connecticut, Storrs, Connecticut.

11. Licensed material shall be used by, or under the supervision of, individuals designated by the Radiation Safety Committee, Hedley C. Freake, Ph.D., Chairman.
12. The Radiation Safety Officer for this license is Kenneth W. Price.

B. Notwithstanding Paragraph A of this Condition, sealed sources designed to emit alpha particles shall be tested for leakage and/or contamination at intervals not to exceed three months.

C. In the absence of a certificate from a transferor indicating that a leak test has been made within six .

months prior to the transfer, a sealed sou'r ce or detector cell received from another person shall not be put into use until tested. l D. Each sealed source fabricated by the licensee shall be inspected and tested for construction defects, leakage, and contamination prior to any use or transfer as a sealed source.

E. Sealed sources and cletector cells need not be leak tested if; (i) they contain only hydrogen-3[oEs ..

when they are removed from storage for use or transfer to another person, and have not been tested within the required leak test interval, they shall be tested before use or transfer. No sealed source or detector cell shall be stored for a period of more than 10 years without being tested for leakage and/or contamination.

F. The test shall be capable of detecting the presence of 0.005 microcurie of radioactive material on the I test sample. If the test reveals the presence of 0.005 microcurie or more of removable contamination, a report shall be filed with the U.S. Nuclear Regulatory Commission and the source or detector cell l shall be removed immediately from service and decontaminated, repaired, or disposed of in accordance with Commission regulations. The report shall be filed within five days of the date the leak test result is known with the U.S. Nuclear Regulatory Commission, Region I, ATTN: Director, Division of Nuclear Materials Safety,475 Allendale Road, King of Prussia, Pennsylvania 19406. The report shall specify the source or detector cell involved, the test results, and corrective action taken.

G. The licensee is authorized to collect leak test samples for analysis by the licensee. Alternatively, tests for leakage and/or contamination may be performed by persons specifically licensed by the Commission or an Agreement State to perform such services.

15. Sealed sources or detector cells containing licensed material shall not be opened or sources removed from source holders by the licensee.

16. The licensee shall conduct a physical inventory every six months to account for all sealed sources and devices containing licensed material received and possessed under the license.
17. Except as specifically provided otherwise in this license, the licensee shall conduct its program in accordance with the statements, representations, and procedures contained in the documents, including any enclosures, listed below. The Nuclear Regulatory Commission's regulations shall govern unless the statements, representations, and procedures in the licensee's application and correspondence are more restrictive than the regulations. '

I am responding to questions you posed today to Mr. Frank Labato, Director of the Department of Environmental Health and Safety (EH&S).

1. Regarding the availability ofMr. Kenneth W. Price to act as Interim Radiation Safety Manager for the University of Connecticut at Storrs, Mr. Price will physically be on this campus for one day per week. He will maintain an office within EH&S, and will

) review all Licensed Investigators' requests for new and renewed protocols; review all completed laboratory survey inspection forms; and perform any other administrative tasks formerly conducted by the Radiation Safety Manager currently lir.ted on the three licenses.

2. ' As concerns the staffing of the Radiation Safety Section within EH&S, a Radiation Safety Specialist position was recently refilled on July 27,1998. This brings the current staffing level to: an Interim Radiation Safety Manager, two full-time Radiation Safety Specialists, a clerical support position, a 20-hour-per-week Graduate Student, and a student labor position.
3. Mr. Labato has advertised the Radiation Safety Manager's position in the August 23 Hartford Courant Sunday newspaper; on the Intemet Lists, Radsafe and Safety; and it will appear in the October issue of the Health Physics Society Nc.ws!ctter. Mr.

Labato anticipates filling the position by the end of the year.

The Radiation Safety Manager has resigned effective August 28,1998, and the University is requesting that an Interim Radiation Safety Manager be named, pending the completion of a search to hire a replacement. The Interim Radiation Safety Manager will function with the full authority of the Radiation Safety Manager presently specified on the license; will have authority to shut down imminently hazardous operations; and will oversee all aspects of the University's R. .liation Safety Program. Regarding coverage during emergency situations, the Interim Radiation Safety Manger wears a pager, and can also be called at home on nights and weekends. His primary place of employment and his home are located cpproximately 40 and 70 minutes, respectively from the University.

August 28,1998. The University of Connecticut requests that License No. SNM-1889 be amended to name Kenneth W. Price as Interim Radiation Safety Manager, pending completion of a search for a replaceaent Radiation Safety Manager.

Mr. Price is currently the Radiation Safety Officer for the University of Connecticut Health Center's Broadscope License No. 06-13022-02 and Specific (Irradiator) License No. 06-13022-04. He is a Certified Health Physicist, Director of Radiation Safety at the University of Connecticut Health Center, and previously worked at Yale University for 16 years as Assistant Radiation Safety Officer and Deputy Director of Radiation Safety. His responsibilities at Yale included work with Special Nuclear Materials.

Mr. Price's resume is on file with the Nuclear Regulatory Commission under the above listed Health Center Licenses.
