Subject: Team Inspection 50-213/88-06 (March 7-11, 1988)

This-transmits the findings of ~an operations safety team inspection led by Mr.

,.

R. Conte of my staff at the Haddam Neck Plant. The inspection team reviewed your

preparations for operation af ter the extended Cycle 15 refueling outage. The in-

spection findings were discussed with Mr._ D. B. Miller and other members of your

staff.

The team concluded that the plant was ready to proceed with mode changes up to full

power operations, assuming 5 tisfactory completion of identified items prior.to

these mode changes. We noted, however, the lack of a consolidated and formal re-

view. process to assure readiness for such startups. We understand that a formal

process is being developed for this. Please let us know if we have any misunder-

standings in this regard.

The team also identified. persistent lack of adherence to administrative control

procedures. Although no safety issues were identified, it is important that this

matter be addressed promptly to assure that it does not lead to a more serious

concern. We~ note that your corrective action systems are viable and should iden-

tify 'any associated conditions which may affect safety; however, you should remain

sensitive.to this persistent problem. ,

No response to this letter is required. Thank you for your cooperation in these

matters.
