. MISSING STEAM GENERATOR TUBE PLUGS The licensee notified RII on June 25, 1977 that ten additional steam generator the 4B steam tube explosive plugs .were missing from the cald leg side of generator.

These plugs are used to seal steam generator tubes that are leaking primary water to the secondary. steam. system.

The licensee had previously reported one plug missing on May 23, 1977, which plate. was subsequently located inside of the tube near the sixth support One of the p16gs had been installed in the steam generator .,

during the outage for steam generator inspections in September 1976 and ten plugs were installed during an outage in November 1976.

The licensee believes the cause of the rissing. plugs to be associated with incomplete detonation of the explosive charges; however, a review of the are current being p1'ugging practices is being made and inspection practices evaluated.

Tube plugging is continuing with emphasis on the use of dry powder of the correct weight.

The licensee has conducteci a preliminary investigation of seven of the tubes and the plugs are not inside. of them.

The licensee notified RII on June 27, 1977 that plans are being formulated for a more extensive l search which may require 1pternal inspection of the reactor vessel. If this is necessary, the cufrent outage may be extended three to four weeks beyond the scheduled July 16, 1977 startup. -

The media has demonstrated interest in steam generator problems in the Past, however, a press release is not planned by the licensee or NRC.
