APPENDIX A NOTICE OF VIOLATION Arkansas Power & Light Company Docket:
50-368 Arkansas Nuclear One, Unit 2 License:
NPF-6 During an NRC inspection conducted during the period July 1-31, 1988, a violation of the NRC requirements was identified. The violation involved failure to position e manual valve as required by procedure.
In accordance with the "General Statement of Policy (1988), the violation is listed below:
and Procedure for NRC Enforcement Actions," 10 CFR Part 2. Appendix C Unit 2 Technical Specification 6.8.1 requires that written procedures be established, implemented and maintained covering tne activities recomended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.
Item 3.d of this Appendix requires procedures for the operation of the emergency core cooling system.
Procedure 2104.39 "HPSI System Operational," has been established in accordance with this Technical Specification requirement.
Attachment A "A SIAS Standby," of this procedure requires that Manual Valve 2SI-1042A be normally closd.
Supplement I, "2P-89A Monthly Test,"
of this procedure also requires that discharge test gauges be isolated at the completion of the monthly high pressure injection pump test by closing Valve 2SI-1042A, Contrary to the above on July 14, 1988, the NRC inspector found that Manual Valve 2SI-1042A was open.
This is a Severity Level V violation.

Pursuant to the provisions of 10 CFR 2.201, Arkansas Power & Light company is hereby required to submit a written statement or explanation to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:
(1)the reason for the violation if admitted. (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
