Gentlemen: This refers to the inspection conducted by Messrs. W. F. Smith, E. A. Plettner, P.'W. Michaud, W. B. Jones, and T. Reis during the period February 11 through March 31, 1988, of activities authorized by NRC Operating License NPF-71 and Construction Permit CPPR-129 for the South Texas Project, Unit 1 and Unit 2, respectively, and to the discussion of ' , .~ :Mings with Mr. W. H. Kinsey and . other members of your staff at the conclusion of the inspection. Areas examined during the inspection included onsitt followup of events, monthly maintenance observation, licensee action on previous inspection findings, monthly surveillance observation, engineered safety teature system walkdown, operational safety verification, and startup testing observations. Within these areas, the inspection consisted of selective examination of procedures and representative records, interviews with personnel, and observations by the NRC inspectors. The inspection findings are documented in the enclosed inspection report. During this inspection, it was found that certain of your activities appear to be in violation of NRC requirements. As discussed in our telephone conversa- tion on April 27, 1988, with you, we have tentatively scheduled an enforcement conference during the week of May 16, 1988, to discuss your understanding of the apparent violations, the reasons for occurrence, and your corrective actions. We will also discuss the NRC Enforcement Policy regarding these matters, if appropriate. The NRC is particularly concerned regarding the violation that occurred February 9,1988, involving operation in Mode 3 with seven out of twelve feedwater flow transmitters out of service. The NRC considers this violation a breakdown in your system turnover and/or startup programs and may be indicative of weaknesses in your ability to ensure that safety systems are at the state of operational readiness required for the progression of operating modes approaching full power operation. You are requested to address all aspects of this concern and provide assurance that there will be no future similar incidents in either Unit 1 or Unit 2. Houston Lighting & Power Company -2- . proper conditions for entering and exiting Technical Specification action statements as discussed in paragraph 2.d of this report. The adequacy of the entire program for lifted leads, fuse removal, and temporary jumpers is also of concern because of the weaknesses identified in paragraph 3.d of this report. Accordingly, the circumstances of the two additional proposed violations may also be discussed during the enforcement conference. We have also examined actions you have taken with regard to previously identified inspection findings. The status of these items is identified in paragraph 4 of the enclosed report.
