e, APPENDIX A NOTICE OF VIOLATION Nebrada Public Power District Docket:

50-298 Cooper Nuclear Station Operating License: DPR-46 During an NRC inspection conducted on' April 4-7, 1988, a violation of NRC requirements was identified.

The violation involved failure to perform QA surveillance to verify that welding is conducted in accordance with procedures.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed below:

Criterion X of Appendix B to 10 CFR Part 50 states, in part, "A program for inspection of activities affecting quality shall be established and executed by or for the organization performing the activity to verify conformance with the documented instructions, procedures, and drawings for accomplishing the activity...." Paragraph 2.10 of the Nebraska Public Power District Quality Assurance Program for Operations - Policy Document, Revision 3, states, in part, "Quality Assurance audits and surveillance of activities such as maintenance, repair, and modifications will include direct observation;...." Welding is a special process required to be conducted by qualified personnel using qualified procedures. This is required by Criterion IX of Appendix B to 10 CFR P;rt 50.

Contrary to the above, Quality Assurance surveillances of safety-related welding activities were not performed to verify by direct observation that significant modifications such as Design Changes87-113 and 87-063 were accomplished by qualified personnel using qualified welding procedures.

This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Nebraska Public Power District is hereby required to submit to this office, within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:

(1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, (3)thecorrectivestepswhichwillbetakentoavoidfurtherviolations,and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
