APPENDIX A Crucible Materials Corporation Trent Tube Division Docket flo. 99902008/88-01 NOTICE OF VIOLATION As a result of the inspection conducted on February 16-19, 1988, and in accordance with Secticn 20E of the Energy Reorganization Act of 1974 and its implementing regulation 10 CFR Part 21, the following violations were identified and categorized in accordance with the NRC Enforcement Policy (10 CFR Part 2. Appendix C), 49 FR 8583(March 8,1984):

Section 21.31 of 10 CFR Part 21 dated October 31, 1986, states, in part.

"Each... corporation... subject to the regulations in this part shall assure that each procurement document for a... basic component... specifies, when applicable, that the provisions of 10 CFR Part 21 apply."

Contrary to the above, a review of purchase orders (PO) to vendors revealed that while 10 CFR Part 21 was imposed upon Trent Tube (TT). TT did not impose 10 CFR Part 21 requirements on P0s 54113 (December 30,1985) to Anderson Laboratories, 50005 (February 27,1985) to Conax, and 58684 (January 22,1987) to Instrumatics (January 22,1987).
