Gentlemen:

This refers- to the routine safety inspection conducted by Messrs.

Stephen Guthrie and Michael Parker of this office on November 1, 1985 -

January 23, 1986, of activities at Big Rock Point Nuclear Plant authorized

by NRC Operating License No. DRP-6 and to the discussion of our findings

with Mr. David Hoffman at the conclusion of the inspection.

The enclosed copy of our inspection report identifies areas examined during

the inspection. Within these areas, the inspection consisted of a sel y tive

examination of procedures and representative records, observations, and

l interviews with personnel.

No violations of NRC requirements were identified during the course of this

inspection. However, this inspection identifies two examples of the type of

incident which resulted in escalated enforcement action during December 1985.

Specifically, an incident of Field Maintenance Service employees failing to

follow local control procedures for component isolation resulted in a valve

being repaired without tagging or isolation and is believed to have resulted

in a steam leak from the reactor vessel and subsequent scram. Additionally,

on two occasions Reactor Operators performing control rod manipulations failed

to record rod notch movements on a laminated card file implemented by you as

corrective action to address two incidents of mispositioned control rods in

1985. Because these events are additional examples of citable offenses which

were thoroughly addressed in an Enforcement Conference December 5, 1985, and

because you are' actively developing and implementing corrective measures

expected to have a positive effect on the conduct of operations at your

facility, these findings are not presented as violations in this report.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter and the enclosed inspection report will be placed in the NRC

Public Document Room.
