PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-7 This preliminary notification constitutes EARLY notice of an event of POSSIBLE safety or public interest significance. The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

SUBJECT:

WELDER IDENTIFICATION - PIPE HANGER AND RESTRAINT WELDS At approximately 10:00 a.m. on April 14, 1977, Region II was advised by an inspector on site that the licensee had determined that certain of the piping restraint and hanger welds did not have proper welder identi-fication stamps as required by VEPC0 erection procedures.

1. Some welds had no identification stamps,

2. Some welds had no identification stamps, but repairs had been stamped.

3. Some welds had identification stamp numbers of welders that had not made the welds.

Region II was further advised of the following

1. The problem came to light after a welder on April 8,1977,

refused to affix his welder identification number to a -

l weld he had not made when asked to do so by a Welding Supervisor. The welder was terminated on the grounds that he had not followed procedurrs controlling weld filler metal. The Welding Supervisor is stated to have realized that he could have made a mistake (in requestin to affix his identification number to the weld)g and the welder reported what he had done to his own supervisor. The Welding Super-visor has been suspended as well as others that have apparently been implicated.

2. The equirement that the weld be stamped with the welder's -

i identification number is not a NRC requirement but is a i VEPC0 procedure requirement.

VEPC0 was made aware of the matter late on April 12, 1977, /' I and advised the Region II inspector late on April 13, 1977.

They reported the matter as an initial notification under 10CFR50.55(e). g,u

4. VEPC0 has determined that quality piping and Heating Ventilation and Air Conditioning duct restraints and -

hangers in both Units 1 and 2 are involved. The' total number of welds has not been determined.

5. VEPC0 is in the process of inspecting all restraint and hanger welds for proper welder identification. ,

6. VEPC0 is not planning a press releake. Any delay that might be occasioned by the problem has not yet been identified.

7. VEPC0 does not know what, if any corrective actions will be required.

8. It is not known at this time if weld quality is involved.

9. Region II has initiated an investigation and plans to question the terminated welder and others that have been i' implicated. An investigator is being dispatched to the

, site today (April 14,1977).
