Receives report of radiological emergency and ootains information on Initial Notification Form (Attactrnent 2).

Telephones appropriate person (f acility) initiating emergency notification and obtains information on Attactrnent 2.

Determines if emergency actually exists and what response is needed.

6.

If warranted, provides contact person at the scene with appropriate precautionary instructions, pending on-site evaluation.

7.

Contacts appropriate response agency,'if other than Department of Health (DOH).

Lead Response Agency Guide ( Attachment 3) may be used.

17. Makes appropriate measurements and collects environmental or wipe samples as needed.

Forwards samples to Laboratory of Radiological Sciences (LRS) or to BERP.

j Completes appropriate information (see Attachttent 5) for all samples sent to the laboratory.

18. Prepares report of emergency, including any noted Code violations and appropriate recommendations. Forwards to BERP, with one copy to operator, if a DOH licensee or regi strant.
19. Inspects f acilities regulated by DOH, as necessary, to determine compliance with Code and/or recornendations.

20. Provides assistance to RRHS or LHU as needed, or responds in lieu of RRHS or LHU, if necessary.
21. Evaluates available information and

requests collection of environmental samples.

22. Coordinates analysis of collected samples with LRS and provides results to RRHS and LHU, as appropriate.
23. Distributes copies of final report to appropriate recipients of the Problem Alert form.
24. Forwards copies of final report to appropriate Federal or State agencies and requests their follow-up with facilities they regulate.
