NRC INSPECTION REPORT #85-45 NOTICE OF VIOLATION HOPE CREEK GENERATING STATION Your letter dated November 19, 1985, transmitted the above referenced Inspection Report which contained a Notice of Violation citing one (1) item of noncompliance concerning excessive spacing between cable restraints on vertical cable runs.
As of September 27, 1985, several safety-related cable trays at floor penetration E-4301-4 had vertical cable spans of six to twelve feet between the penetration seal and the first tie to a cable tray rung.
Corrective Steps Taken and Results Achieved As detailed in the subject Inspection Report, the immediate corrective actions for the identified violation were completed during the inspection period and verified by the NRC Inspector.
Corrective Steps Taken to Preclude Recurrence At the time of cable installation, cables were secured to vertical cable trays at five foot intervals without regard to proximity of floor penetrations.
Consequently, it became necessary to remove some ty-raps located near floor penetrations in order to separate cables for proper application of penetration sealing 103Og f 1, \\

On May 17, 1985, a walkdown was initiated to identify all penetrations where ty-raps had been, or would be, cut to facilitate penetration sealing.
Where penetrations had been sealed, ty-raps were reinstalled to the requirements of drawing E-1408 and inspected by Quality Control.
At penetrations which had not been sealed, ty-raps were reworked, as required, to allow for penetration sealing, yet maintain the required five foot spacing.
Subsequent to NRC Inspection No. 85-45, additional examples of excessive ty-rap spacing at floor penetrations were identified by the NRC, PSE&G and Bechtel.
These additional cases were documented on Nonconformance Reports (NCR) 8493, 8847 and 8786.
To determine the extent of the problem, an investigation was performed which included a review of the program for control of cable installation and penetration sealing activities.
The investigation determined that some craft personnel had not received sufficient training on the requirements of SWP/P-E-17, and had cut ty-raps in the process of preparing penetrations for sealing.
In order to identify and correct all such cases, a walkdown of all floor penetrations containing safety related cable sealed after May 17, 1985, will be performed.
Any violations of ty-rap spacing found will be documented on Nonconformance Reports.
Training on the requirements of SWP/P-E-17, which provides for rework of ty-raps when necessary, was provided to the applicable craft and supervisory personnel.
In addition, the applicable Quality Control Engineers will receive training reemphasizing the requirements of Project Quality Control Instruction (PQCI)
E-4.2.
Specifically this training will direct that inspections of ty-rap rework activities be performed only after the penetration is sealed.
Date of Full Compliance The training of craft and supervisory personnel was completed on November 19, 1985, and the training of Quality Control personnel was completed on December 13, 1985.
The walkdown of floor penetrations will be completed by January 31, 1986.
