During the 6-week period covered by this inspection, operations, maintenance, and

engineering activities were generally performed in a satisfactory manner. Operability

training, a problem identified in past NRC inspections, was effective in providing operators

with a uniform understanding of how the operability determination process is conducted.

However, the containment equipment inner door was lef t open on May 30, and in spite of

')

continuous tours by operators every 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br />, this condition was not identified until 20

hours later. Also, our review of several CRD surveillance tests identified several that were

left with identified insertion speeds outside of procedural requirements. These events

,

highlight the need to provide continued focus and attention by plant staff to assure that

plant conditions and components remain within procedural requirements.

Based on the results of this inspection, two violations of NRC requirements were

identified. The first violation involved a failure to follow procedures during control rod

drive insertion time testing. The second violation involved a failure to take prompt

corrective action when the containment equipment-lock inner door was found open. The

violations are cited in the enclosed Notice of Violation, and the circumstances surrounding

the violations are described in detail in the enclosed report. Please note that you are

required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response. The NRC will use your response, in part,

to determine whether further enforcement action is necessary to ensure compliance with

regulatory requirements.
