NRC INSPECTION REPORT 50-454/97008(DRP); 50-455/97008(DRP) AND

Dear Mr. Graesser:

On June 12,1997, the NRC completed an inspection at your Byron 1 & 2 reactor
l
facilities. The enclosed report presents the inspection results.
l

During this 6-week inspection period, the inspectors observed excellent operator
performance during two Unit 1 shutdowns for equipment repairs. Preparations to reduce
power and shut down Unit 1 and Unit 2 due to a missed technical specification (TS)
i
l
surveillance were also well planned and executed. Heightened level of awareness briefings
~
held prior to all shutdowns continue to be thorough with emphasis on procedural
i
compliance, control of personnel in the control room, reactivity control, and lessons
learned.
Based on the results of this inspection, certain of your activities appeared to be in violation
of NRC requirements. The inspectors identified three violations. The most significant
dealt with preconditioning the 28 diesel driven auxiliary feedwater pump engine by running
the ASME surveillance prior to the slave relay start surveillance. Although the inspectors
considered the preconditioning incident to be isolated due to inadequate test scheduling,
such an activity might have an adverse effect on the validity of test results. The second
violation involved the failure to perform the corrective actions stated in an LER, and the
third violation involved not utilizing the temporary alteration process for installing a strip
chart recorder attached to a safety-related 125V dc battery charger.

The violations discuss above are cited in the enclosed Notice of Violation. The

circumstances surrounding the violations are described in detailin the enclosed report.
i

instructions specified in the enclosed Notice when preparing your response. The NRC will

use your response, in part, to determine whether further enforcement action is necessary
to ensure compliance with regulatory requirements.
