This report is submitted in accordance with Byron Station Unit 1 Operating License NPF-37 Appendix A, Technical Specification 4.8.1.1.3, reporting of diesel genccator failures.
This report addresses one valid test and failure experienced on the IB Diesel Generator (DG). The criteria to determine valid tests and failures is in accordanco with rection C.2.e of Regulatory Guide 1.108.
Attachment A, DVR 6-1-88-018, is a sumary of the current 1B DG failure. This failure is the second failure within the last 20 valid tests for the IB DG, therefore, the IB DG has been placed on a weekly test interval. This failure was also the seventh failure within the laat one hundred Unit 1 DG valid tests. Therefore, this letter is supplemented with the additional information requested by Regulatory Guids 1.108 C 3.b. is a sumary of all tests that occurred within the time period over which the last one hundred valid tests were performed.
Corrective actions have been taken, and are planned, to increa.se the reliability of the Unit 1 diesel generators. Four of the seven failures during the last hundred valid tests were the result of slow engine starts. A major contributor to the slow starts has been a loss of fuel prime.
To correct this problem, soft seat check valves were added in the fuel oil supply line. In addition, a modification to reroute the fuel oil supply line with a loop seal, and a anti-syphon vent will be completed during the 9/88 refueling outage.
Two of the seven failt.ces have been caused by intermittent operation of a
)
relay used in the governor control circuit. A modification to replace the subject relay with a more sensitive relay is currently being designed. This reodification will be implemented during the 9/88 refueling outage. The other failure was due to a fuel oil leak at a fuel injection pump. No long term

corrective actions were taken as a result of that event as the event was considered an isolated failure.

Byron Station has stated before, and still believes, that the overall a
reliability of electric power to engineered safety feature equipment is adequate. The primary source of electric power, the Unit 1 system auxiliary transformers and associated breakers, have experienced 100% reliability since receipt of the Unit 1 operating license. The backup source of offsight power,the Unit 2 system auxiliary transformers and associated breakers, have experienced three unrelated failures during this same period.
In all cases the Unit 2 diesel generators functioned per design. The reliability of offsite power and the associated transformers and breakers, in addition to the 3
confidence Byron Station has in the future operation of the emergency diesel generators, provides the basis for continued plant operation.

On 1/28/84, at 1124 with Unit I in Mode 1 at 981 Reactor Power, the 1B Diesel Generator (EK](DG) was started per monthly ByrrA Operability surveillance, 180$ 4.1.1.2.a-2, following a governor oil change. A successful maintenance run had been perforacd ineuediately preceding the operability run. Following a successful start, the Nuclear Station operator (NS0) (Licensed) closed the output breaker and the IB OG-j began to load uncontrollably. Several attempts were made to reduce the KW loading with the governor j
control switch, however none were successful. The IB OG output breaker was manually tripped open f rom the 1
main control board switch when the generator load reached 5800 KW.
Limiting Condition for operation Action Rtquirement (LCOAR) 180$ 8.1.1-13 had been initiated prior to the governor oil change. therefore, no further actions were required. There were no systems or components inoperable prior to this f ailure that contributed to the failure.
Due to previous experience with failures of this nature. the 4tX3 relay was removed and bench tested. The 4EX3 relay aligns the electronic governor with a fixed resistor in the energency nuode for a constant speed reference. In the testing mode the relty lines up a motorized potentiometer for variable speed reference.
Troubleshooting revealed a slight resistance across the normally closed contacts (test mode contacts) and j
significant resistance across the normally open contacts (emergency g de contacts) once the relay was j
energized. Due to this inconsistent operation of the relay, the relay was replaced. The replacement relay was cleaned nd bench tested to insure proper operation.

Following the replacement of the subject relay. the IB DG was started for a asintenance run. The output breaker was closed and the diesel was operated at various loads for approximately thirty minutes. No abnormal conditions were detected. At 2039 on 1/28/88, the 18 OG was restarted for the monthly operability surveillance. $1x minutes into the run. load oscillations of 200-400 KW were observed by the M50.
Electrical maintenance personnel were notified and troubleshooting was again initiated. With the diesel running in a loaded condition a jumper was placed across the normally closed contacts on the 4EX3 relay.
The junger eliminated the possibility that the newly installed atX3 relay was the cause of the load oscillations. The large load swings continued with tie jtsaper in place so the IB DG was shutdown at 2227.
During this 108 minute run load oscillations ranged from 200-1000 KW.
Another maintenance run was performed at 2345 with no further conclusions betrg made.
Based on a review of a similar occurrence at Braidwood Nuclear Power Station (Docket Number 456) (LER 87-039) the IB OG was restarted for a troubleshooting run at 0457 on 1/28/88. A six channel strip chart recorder was connected across several locations within the altetronic governor to monitor governor l
performanca. Data was collected and the DG was shutdown at 0604 Due to the Brat hood experiences with OG load swings, two tra1 &ood engineers were brought to the site to assist in the investigation. In addition, a Cooper Bessemer governor representative was contacted for assistance. Based on discussions with all personnel involved three sure actions were taken. The current transformers were lamped to verify circuit integrity. No abnormal conditions were found. All terminal connections within the electronic governor were checked for tightness and again no abnormal conditions were found. The IB 04 was again started at 1640 for further data c311ection and based on a review of all data collected, it was decided to replace the standard load sensor module within the electronit, governor.
Following replacement of the standard load sensor module the it OG was restarted at 1200 on 1/29/88 for a maintenance run. During this run, proper operation of the load sensor was verified. In addition, p M r operation of the 4EX3 relay the 3!M relay, and the motorized potentiometer were verified. One small load swing was observed during a large voltage aspere reactive (VAR) adjustment from the control room. A conversation with the veltage regulator specialist at the Ccs.lonwealth Edison Technical Center revealed that the load swing was not an abnormal reaction to the VAR swing.
The maintenance run was terminated at 0005 on 1/30/88. Following an extensive review of the data collected. the troubleshooting performed, and the corrective actions taken, it was 1ecided to start the it OG per the monthly operability surve11)ance. During the surveillance, no load oscillations were observed.
At 0600 on 1/30/88, the surveillance was successfully con 91sted and the LC0AR was esited.

j The root cause for this failure is twofold. The initial failure on 1/28/84 at 1124, was caused by a faulty
{
4EX3 relay and intermittent operation of the standard load sensor. The load swings experienced following the replacement of the 4EX3 relay are attributed to intermittent operation of the standard load sensor only. Due to the inconsistent resistance values observed during bench testing of the removed 4EX3 relay.
it cannot be determined if the resistance would have af fected the DG in the emergency mode. Ti.orefore, per Regulatory Guide 1.108, section C.2.e. this failure has been deemed a Valid Test and Failure.

The standard load sensor and the 4EX3 relay were renoved and replaced. The Diesel Generator war. tested and declared operable at 0600 on 1/30/88. Pursuant to Technical Specification 4.8.1.1.2.a. the 18 OG has been placed on a weekly test interval.
