ATTACHMENT I Violation:

Technical Specificatio_n 6.11 states that "procedures.for personnel radiation protection shall be prepared consistent with the requirements of 10 CFR 20 and shall be approved, maintained and adhered to for all operations involving personnel radiation exposure."

Procedure 9300-ADM-4110.04, Radiation Work Permit (RWP), section 7.4.1, requires in part, that af ter reading the RWP the worker shall complete RWP Attachment Sheet (Attachment 10.3) signifying that the individual has read, understands, and will comply with the radiological requirements specified.

Contrary to the above, on May 4,1988, a Quality Control inspector was observed by an NRC inspector inside a posted "Contaminated Area" on the 119 ft. elevation of the reactor building while not dressed in accordance with the minimum anti-contamination clothing requirements of RWP No.

880335, "Channel New Fuel /Rechannel Old Fuel " Spect fically, the QC inspector entered the contaminated area without a surgeon's cap, cloth hood, one pair coveralls, rubber gloves, or cloth shoe covers.

The controlling RWP required anti-contamination clothing to include, as a minimum, a surgeon's cap, cloth hood, one pair coveralls, cotton glove liners, multiple pair rubber gloves, and two pair cloth shoe covers.

Response

GPUN concurs with the finding stated in the above Notice of Violation.

The following corrective actions have been taken:

1.

The individual involved was restricted from the Radiologically Controlled Area, was counseled, and following a successful interview with the Radiological Controls Field Operations Manager, was allowed to return to work.

2.

The area in which the violation occurred was reposted to ensure i

clarity as to the actual boundaries of the contaminated area.

The event was reviewed with station staff at the Plan of the Day meeting on May 5,1988.

This meeting is attended by management and representatives from virtually all site departments involved in plant work that has a potential for contact with radiological contamination.

These actions were designed to avoid further violations of the nature described in the Inspection Report 88-13.

Full compliance was achieved by May 12, 1988.
