Subject: Combined Inspection Report No. 50-289/88-12 and 50-320/88-09
A routine, unannounced, safety inspection was conducted b three Region I
inspectors, S. Sherbini, J. Gresick, and R. Loesch, on Na 30 - June 3, 1988.
The inspection effort was concentrated on the Unit 2 radi logical controls
program, as well as those elements of the site radiological controls program
that are common to Units 1 and 2.
Within the scope of this inspection, two violations were identified. One
violation involved failure to perform an adequate survey, and the second
violation involved failure to follow procedures. Details are provided in the
enclosed Appendix A and inspection report. You are required to respond to the
violations in accordance with the instructions contained in the enclosed
Appendix A. We are also concerned about your management control system that
allowed frequent incidents to occur that were caused at least in part by lack of
adherence to 3rocedures. You are therefore recuested, in addition to your
response to tie violations described in Ap)encix A, to inform us of the steps
you have taken to improve performance in t11s area.
