Gentlemen:

SUBJECT: NOTICE OF VIOLATION (NRC INSPECTION REPORT NOS. 50-498/88-11

AND50-498/88-24)

This refers to the inspection conducted February 11 through March 31 and

April 5 through May 2, 1988, at the South Texas Project, Unit 1 (STP-1) near

Bay City, Texas.

During these inspections, it was found that certain of your activities appeared

to be in violation of NRC requirements. On May 26, 1988, an enforcement

conference was held in the Region IV office in Arlington, Texas, to discuss

violations identified during recent NRC inspections (reference NRC Inspection

Report Nos. 50-498/88-11 and 50-498/88-24). Two of the apparent violations

(Violations 498/8824-03 and 498/8811-01) are under review and will be addressed

in a separate letter. One apparent violation (498/8811-04) involving not

having a specific procedure to control lifted leads has been withdrawn based on

information you provided during the enforcement conference. The remaining four

apparent violations are contained in the attached Notice of Violation.

Violation A in the enclosed Notice of Violation involves deficiencies identified

by the NRC during a test results verification inspection. This violation is

potentially significant because these packages had been through several reviews

in your system and the errors had not been discovered. However, these

deficiencies were not significant because the errors were isolated and had no

impact on the successful completion of the test.

Violation B represents a potentially significant weakness in adherence to your

Technical Specification (TS) requirements. During power operation it is

important that operations staff personnel be aware of all system operability

requirements prior to exiting TS Action Statement.

Violation C represents a potentially significant weakness because of the failure

to properly control system boundaries in accordance with procedural requirements.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response. In responding,

you may refer to previous correspondence to the NRC where you may have

documented all or part of your required response to these violations. After

reviewing your response to this Notice, including,your proposed corrective

actions and the results of future inspections, the NRC will determine whether

further NRC enforcement action is necessary to ensure compliance with NRC

regulatory requirements.

In accordance with Section 2.790 of the NRC's "Rules of Practice," Part 2,

Title 10, Code of Federal Regulations, a copy of this letter and its enclosure

will be placed in the NRC Public Document Room.

The responses directed by this letter and the enclosed Nctice are not subject to

the clearance procedures of the Office of Management and Budget as required by

the Papemork Reduction Act of 1980, Pub. L. No. 96-51'..
