SUBJECT: NOTICE OF VIOLATION
(NRC INSPECTION REPORT NOS. 50-259/88-02, 50-260/88-02, AND
50-296/88-02)
This refers to the Nuclear Regulatory Commission (NRC) inspection conducted
by G. L. Paulk on January 1-31, 19S8. The inspection included a review of
activities authori:ed for your Browns Ferry facility. At the conclusion of
the inspection, the findings were discust,cd with those members of your staff
identified in the enclosed inspection report.
Areas examined during the inspection included operational safety, maintenance
observation, restart test program, maintenance improvement program, management
meetings, Surveillance Instruction upgrade program, joint test group meetings,
and engineering changes and modifications. Within these areas, the inspection
consisted of selective examinations of procedures and representative records,
interviews with personnel, and observation of activities in progress.
The inspection findings indica,e certain activities appeared to violate NRC
re q ui rerae nts .
The violation, references to pertinent requirements, and
elements enclosed Notice of Violation.
An unresolved item is identified in paragraph 9, regarding deficient controls
over the annual FSAR updates which have resulted in an FSAR that cannot be
relied upon for 10 CFR 50.59 purposes.
This matter will be pursued during
future inspections.
In addition to the need for corrective action regarding the specific matters
,
identified in the enclosed notice, we are concerned about the implementation
of your engineering changes and modifications programs.
Twelve important
findings are described in the report and summarized in paragraph 11.c.
Consequently, your response shoulo describe those particular at tions taken or
planned to improve the effectiveness of your program.
