Dear Mr. McCarthy:

On June 28 and 29, 1988, NRC senior managers met to review the performance of operating nuclear power plants licensed by the NRC. These meetings are conducted biannually to focus NRC resources on those plants and issues of greatest safety significance. At this meeting, the Fermi facility was categorized as requiring continued close monitoring. Plants in this category have been identified as having weaknesses that warrant increased NRC attention from both headquarters and the regional office.

A summary of NRC discussions held relating to Fermi is provided below:

The most recent SALP assigned ratings of "3" to the areas of operations, maintenance, surveillance, engineering and technical support, and quality programs.

Personnel errors in these areas have also resulted in several escalated enforcement cases. Significant management changes have been made.

Improvement programs are in place including one to complete a review of Technical Specifications against the FSAR and the as-built plant; and one to complete a review of operating, surveillance, maintenance, and administrative procedures.

These are scheduled to be completed in December 1988. Programs to upgrade licensed operator training and professional conduct in the control room have met with some success yet operator errors continue to occur, especially during periods of plant shutdowns. Operators, as well as other plant personnel, have had problems using and complying with procedures. Detroit Edison has made improvements in the preventive maintenance area to upgrade the content, scope, and administrative control of preventive maintenance.

Some perfomance improvements in the past six mcnths are evident based on an improving trend in NRC performance indicators, the results of inspections, monthly meetings with the licensee, and close scrutiny of operational events.
