99900335/78-02 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on September 11-14, 1978, it appeared that certain of your activities were not conducted in full occurence with NRC requirements as indicated below:

10 CFR 50, Appendix B, Criterion II, states in part, "The applicant s

assurance program which complies with the req (ime,... a quality shall establish at the earliest practicable irements of this appendix.

This program shall be documented by written policies, procedures or instructions.... The program hall provide for in-doctrination and training of personnel performing activities affecting quality as necessary to assure that suitable proficiency is achieved and maintained."

ASME Code,Section III, Article NCA-4000, Subarticle NCA-4134.2 states in part, "... The program shall provide for indoctrination and training of personnel performing activities affecting quality as necessary to assure that suitable proficiency is achieved and maintained."

Contrary t'o the above, except for NDE personnel, the Boeing cuality assurance program does not provide for indoctrination and training of personnel (e.g. QA engineers, engineering personnel, purchasing personnel, auditors, etc.) to assure that suitable proficiency is achieved and maintained.

10 CFR 50, Appendix B, Criterion VI, states in part, "fieasures shall be established to control the issuance of documents, such as instructions, procedures and drawings...." These measures shall assure that documents, including changes, are reviewed for adequacy and approved for release by authorized personnel and are distributed to and used at the location where the prescribed activity is performed."

BEC-QAM, Section 4.0, paragraph 4.3, states in part,

... The Integrated Record System (IRS) shall be used to control code items and parts during fabrication...."

"The Integrated Record System consists of the following:

Integrated Record System Order (IRS 0)... lists material..., the work to be accomplished, and the examinations, test and inspection operations to be performed in a sequence of operations.

The procedure number and applicable revision number will be identified."

Contrary to the above, the inspection revealed that twenty (20) of the forty-five (45) IRS 0s reviewed did not identify the nondestructive examination and/or welding procedure numbers and revisions.
