SUMMARY

Scope: This routine, announced inspection involved a review and evaluation of the licensee's emergency preparedness program.

Results: A breakdown was found in the emergency ' response training program.

Within the areas inspected, the following violations were identified:

- Failure to provide initial and annual retraining to the Emergency Control Organization, Paragraph 6.

Failure to include as a minimum a test of the conmunication links and notification procedure, for early warning of the public between the site and State and local emergency units, Paragraph 5.

- Failure to provide the opportunity for local emergency assistance personnel to annually tour the facility, Paragraph 3.

- Failure to perform instrument calibration of the rneteorological temperature reader, Paragraph 4.

3. Offsite Support Agencies (88050)

The inspector held discussions with -licensee management reprosentatives-regarding the coordination of emergency. planning with offsite agencies.

Written agreements existed with Lynchburg General-Marshall lodge Hospital,  ;

Concord Rescue' Squad, and Loncord Volunteer Fire Departuent, as specified. l in the Radiological Contingency Plan (RCP). The agracments were updated in l August 1987. l The inspector determined through discussion and a review of records that 'l the licensee was not providing .the opportunity for local emergency )

assistance personnel to tour the facility as requPed by the RCP. The l list formal tour of the facilities for local emergency ~essistance personnel was conducted in December 1986. This was identified to the licensee as a violation.

Violation: (70-824/88-04-01) Failure to provide the opportunity for local emergency assistance personnel-to annually tour the facility.

4. Emergency Plans, Procedure, Facilities, and Equipment (88050)

The inspector selectively examined emergency kits / equipment.- The kits appeared to be mainta,ined in accordance with procedural requirements.

Examination of the meteorological temperature recorder (B&W Serial

' Number 1000966) calibration records ' indicated that -the instrument was last 1987, and was due for calibration December 1987 calibrated on May).15, Procedure RL-IP-1713-01, "Measuring Equipment Control (every six months and Calibrating System," dated August 4, 1987, requires the meteorological-system instruments to be calibrated every 6 months therefore, the instrument was not in calibration. This was identified to the licensee as a viola; ion.

Violation: (70-824/88-04-02) Failure to perform periodic meteorological instrument calibration as required by procedure (RL-IP-1713-01).

One violation was identified.

5. Tests and Drills (8$050)

Discussions with the licensee personnel and a review of records confirmed that tests and drills were conducted in accordance with Sections 7.0 of the RCP. The inspector discussed the communication links and notification

• procedure for early warning between the site and state and local emergency units. Although the licensee periodically verifies the emergency telephone numbers for the State and local emergency units, the licensee indicated that during the annual drill a test of the communication links and notification between the site and State and local emergency units was not performed. The licensee also indicated that this notification had not been done in previous annual drills. This was identified to the licensee as a violation.

Violation: (70-824/88-04-03) Failure to perform a test of the communication links and notification procedure for early warning of the public between the site and State and local emergency units during the annual drill.

One violation was identified.

[ 6. Emergency Organization Training (88050)

The RCP in Section 7.2 Training states that members of the Emergency Control Organization will be retrained annually. The commitment is also made that each new employee will be indoctrinated in his assigned responsibility in time of emergency. Interviews of licensee personnel and a selective review of training records indicated multiple examples of a lack of initial as well as annual retraining for members of the Emergency Control Organization. Two Emergency Officers had been appointed to the position without receiving any specialized training for that position.

The other Emergency Officer had not received annual retraining. The receptionist, as well as the alternates for that position, had not received

3 initial as well as annual retraining. Interviews with two of the F.mergency Officers revealed the following: (a) one did not know where to find or how to use the Protective Action Recommendations; (2) both.did not know that it was the En.ergency Officer's decisional responsibility for initiating site evacuations; (3) one did not know how many accident classifications there were for the site; and (4) one chose the incorrect Emergency Procedure for an injured contaminated personnel accident. The interviews revealed a major RCP and procedure knowledge deficiency. This was identified to the licensee as a violation.

Violation: (70-824/88-04-04) Failure to provide annual retraining for members of the Emergency Control Organization.

One violation was identified.

7. AccidentAssessment(88050)

The inspector reviewed the assumptions and methods used to calculate dose from the range of postulated accidents. The doses from these accidents have been put int.o tables. Current dose assessment methods did not allow for a range of source term isotopes nor did they allow for changing meteorological (real time) data input. Licensee representatives agreed to review this matter and implement appropriate corrective actions.

Inspector Followup Item (70-824/88-04-05) Develop a procedure to perform emergency radiological dose assessment using meteorological and source term input.

No violations or deviations were identified.

8. Exit Interview The inspection scope and findings were summarized on May 27, 1988, with those persons indicated in Paragrapi, 1 above. The inspector described the areas inspected and discussed in detail the inspection findings. The licensee did not identify as proprietary any of the material provided to -

or reviewed by the inspector during this inspection. Dissenting comments were not received from the licensee. On June 1,1988, Region 11 management and licensee representatives had a conference call to discuss the NRC concern that several basic elements of the RCP were not being implemented. The NRC discussed the need for additional high level licensee management attention to ensure that improvements are made in the emergency preparedness area. The licensee discussed the receid personnel changes and reassignment of job functions which resulted in some oi ihe RCP program findings. The licensee representative committed to placing more management emphasis on the program to improve the program area.
