APPENDIX A NOTICE OF DEVIATION Public Service Company of Colc rado Docket: 50'267 Fort St. Vrain Nuclear Generating Station Operating License: DPR-34 Based on the results of an NRC inspection conducted on May 1-31, 1988, a deviation of your commitments was identified. The deviation consisted of not providing adequate purge gas to control rod drive mechanism. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C (1978), the deviation is listed below:

The licensee, by letter to the NRC Region IV dated July 10, 1985, comaitted to compliance with the interim Technical Specification, Limiting Condition for Operations (LC0) 3.1.1.C, and to surveillance requirement (SR) 4.1.1. A.2 which require the following:

Maintain a helium purge flow to each control rod drive penetration when reactor pressurc is above 100 psia.

Verify at least w 'e per 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> that the purge flow is maintained to each control rod arive mechanism when reactor pressure is above 100 psia.

In deviation from the above commitment:

The NRC inspector observed on May 10 and 19, 1988, with reactor pressure greater than 100 psig, that the helium purge flow indicated to the subheaders on flow instruments FI-111268-3, -4, and -7 was reading zero or belcw. Also, the flow to individual control rod drive penetrations indicated zero or below zero.

The reactor building equipment operator's log sheet only required that subheader flow be greater than zero which does not account for instrument error or assure that a helium purge design flow at full density helium of 5.5 lbs/hr per penetration will be maintained.

Public Service Company of Colorado is hereby required to submit a written statement or explanation to this Office within 30 days of the date of the letter transmitting this Notice. This reply, should include for each deviation:

(1) the reason for the deviation if admitted, (2) the corrective steps that have (3) the corrective steps that will be taken buentakenandtheresultsachieved}thedatewhenfullcompliancewillbe to avoid further deviations, and (4 achieved. Where good cause is shown, consideration will be given to extending the response time.
