Crane Midwest Fittings Docket No. 99900014/78-02 ENCLOSURE NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on August 21-24, 1978, it appeared that certain of your activities were not conducted in full accordance with NRC requirements as indicated below:
A.
Criterion XVIII of Appendix B to 10 CFR 50 states in part, "...
Follow-up action, including reaudit of deficient areas, shall be taken where indicated."
Section 11 of the QA Manual, paragraph 11.2.5, states in part, "When discrepancies are identified, a follow-up audit will be performed within 30 days to insure that corrective action has been accomplished by the responsible department."
Contrary to the above, follow-up audits of identified discrepancies were not performed within 30 days as evidenced by the following:
1.
Audit of Section 8 of the QA Manual performed on May 10, 1978, follow-up audit of discrepancies performed on July 25, 1978.
2.
Audit of Section 9 of the QA Manual performed on July 20, 1977, follow-up audit of discrepancies perfonned on July 25, 1978.
- 3. ' Audit ~ of Section 10 of the QA Manual perfcrmed on December 20, 1977, follow-up audit of discrepancies performed on July 25, 1978.
B.
Criterion V of Appendix B to 10 CFR 50 states in part, " Activities affecting quality shall be prescribed by documented instructions procedures or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures and drawings...."
Section 6 of the QA Manual, paragraph 6.1.2, states, "The Inspection Report referenced shall be noted on the Shop Order Traveler adjacent to the operation which revealed the nonconformance."
Contrary to the.above, Inspection Report No. 2513 dated April 26,.1978, was not noted on Shop Order Traveler C-412A X-1 adjacent to the opera-tion which revealed the nonconformance.
In addition, the Shop Order
. Traveler did not identify the weld procedure used to repair weld on under-cut condition.

Criterion XV of Appendix B to 10 CFR 50 states in part, " Measures shall be established to control materials, parts or components which do not -
conform to requirements in order to prevent their inadvertent use or installation...."
Section 6 of the QA Manual, paragraph 6.2, states in part, " Nonconforming materialsLis identified by attachment of a red Rejection Tag and is placed in a segregated area to prevent its use and/or intermingling with conforming materials...."
Contrary to the above, items on Shop Order C548a, Parts Nos. X1, X2,
. l-and X4, identified as nonconforming on Inspection Report No. 2670, did not have a red rejection tag attached and were intermingled with con-j forming materials.
It was noted that red rejection tags for the above 1
parts-were placed in the shop order traveler packages.
