APPENDIX A NOTICE OF VIOLATION Gulf States Utilities Docket:

50-458 River Bend Station License: NPF-47 During an NRC inspection conducted on January 1 through February 15, 1988, one violation of NRC requirements was identified.

The violation involved a failure of timely follow-up review to verify corrective action completion.

In accordance with the "General Statement of Policy (and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C 1987),theviolationislisted below:

Failure of Timely Follow-up Review to Verify Corrective Action Completion Criterion XVI of Appendix B to 10 CFR Part 50 requires that measures be established to assure that conditions adverse to quality, sucn as nonconformances, are promptly identified and corrected.

River Bend Quality Assurance Directive QAD-16, "Corrective Action,"

Section 4.5, requires timely follow-up reviews by the appropriate department to verify that specified corrective action has been properly implemented.

Contrary to the above, on January 21, 1988, specified corrective action had not been completed for a nonconforming condition identified by the licensee on April 12, 1986. The nonconforming condition was that the actual field wiring configuration for the standby gas treatment system recirculation dampers did not agree with the electrical schematic drawings.

The specified corrective action not completed was to implement Modification Request 86-0642 to modify the field wiring configuration.

Timely follow-up review was not conducted by the appropriate department to verify that specified corrective action had been properly implemented.

This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:

(1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to extending the response time.
