Richmond Inserts to Embedded Plates and Welded Attachments to Embedded Plates and All Concrete Attachments" Gentlemen:

On June 11, 1985, by our report logged TXX-4492, we notified you of an inadequacy in construction procedures which we deemed reportabN under the provisions of 10CFR50.55(e).

Installation procedures did not specifically require minimum spacing of concrete anchors (lliiti Bolts, Richmond Inserts) from embedded angles.

This created the potential for installation deficiencies which could compromise the structural integrity of all concrete anchors installed in proximity to embedded angles.

This issue was subseqently identified by our letter logged TXX-6435, dated May 13, 1987, as an issue for which a final report had been issued.

Ilowever, the corrective actions were funnulated but the schedule for the implementation of those actions was not established.

This report is submitted to status the implementation of those corrective actions.

The corrective actions as noted in our report IXX-4492 have been superseded by the corrective actions stated in our report TxX-88185, dated February 5, 1988.

Our final report (Txx-88185) for SOAR CP-86-04, (see reference) included the corrective actions required to resolve all issues concerning embedded angles.
