Subject: Millstone 3 Special Inspection 50-423/88-03 (1/19-29/88)

The enclosed report transmits the findings of the January 19-29, 1988 special in-

spection concerning an overpressure event at low temperature. These findings were

discussed by Mr. W. Raymond of this office with Mr. C. Clement of your staff on

February 18, 1988.

On January 19, 1988, the NRC was informed of the everpressura ' event and your in-

tention to investigate it. On January 22, 1988, our inspector found that the re-

quired overpressure protection systems had not been available to protect against

the transient. This event involved exceeding a Limiting Condition for Operation .

(LCO) and an actual challenge to the unavailable overpressure protection systems.

The operators incorrectly believed that the power operated relief valves (PORVs)

had been operable at their low temperature overpressure setpoint since 9:10 p.m.

on January 16, 1988. This inspection found that no automatic actuation low tem-

perature overpressure protection (LTOP) equipment was available on January 19, 1988

for 16 minutes during which an overpressure condition occurred. Prompt and effec-

tive operator action mitigated the event. 10 CFR 50 Appendix G limits were not

exceeded. Returning one operable Residual Heat Removal (RHR) suction relief valve

to service after 16 minutes provided one low temperature overpressure protection

feature. However, full compliance with the LTOP LC0 was not achieved for four

hours and 40 minutes, when the low temperature overpressure relief setpoint was

made operable for the PORVs. We are concerned with the event itself and the lack

of equipment control and status indication that contributed to it. Accordingly,

we are scheduling an enforcement conference to discuss this event with you in the

NRC Region I Office in King o.f Prussia. We will forward an appropriate notifica-

tion of enforcement action after the enforcement conference.
