SQN SQA201 Page 1 Revision 0 l.0 PURPOSE To provide a basis for review of changes, tests *, or experime$ts described in Tech Spec Appendix B to determine if an Unreviewed Environmental Question (UEQ) exists.*'
2.0 SCOPE This procedure applies only to workplans, TACF's and tests covered by AI-9, AI-19 part IV, and STI All other types of plant procedures have been reviewed and will not cause an UEQ to exist.

3.2 References 3.2.1 AI-9 3.2.2 AI-19, part TV 3.2.3 STI-l 4.0 INSTRUCTIONS 4.1 All workolans, temporary alterations, and special tests that involve the cumping or potential spilling of wastes (petroleum products, chemicals, coolants, etc.), removal of asbestos insulation, earthwork (digging, grading, hauling, disposing of, etc.), new buildings (reworking land), and demolition must be reviewed by the Environmental Section.
If the modification, temporary alteration or special test connects to a potabl.e or raw water system, sanitary sewer, or crain, it would require a review to evalua:e its impact.

SON 5Q4201 Page 2 Revision 0 4.0 INSTRUCTIONS (continued) 4.2 If this review confirms that the work will not result in any new or increased discharges to the air, land or water environment that have not been previously evaluated or covered by environmental permits or the final environmental statement, it shall be determined to be covered by the NEPA STANDARD'S Categorical Exclusions.
If the work does not fall within the categorical exclusions, an evaluation of significance must be made to determine if an UEQ exists.

Dear Mr. Stewart:
SEQUOYAH NUCLEAR PLANT (SQN) - FISHKILL IN SQN DIFFUSER POND - JULY 12 Approximately 1500 dead _ fish were observed in the northwest corner of the diffuser pond on July 12.
The tish ranged from 1 to 20 inches in length.
A species estimate was made and it was determined that approximately 50 percent of the fish killed were carp, 35 percent were catfish, 10 percent were sunfish, 3 percent were drum, and 2 percent were shad.
Samples of water flowing into the diffuser pond from discharge serial No. (DSN) 103 were collected on the date of the kill.
Samples were also collected in the diffuser pond along the bank, 100 feet east of the DSN 103 discharge point.
Results of the analyses were:
DSN 103 Discharto Diffuser Pond Temperature. 'F 86.0 104.0 pH 6.9 7.0 Dissolved oxygen, m;/L 4.2 3.6 Chlorine 0.15 0.10 The chlorine was analyzed by a field Hach kit and results are questionable.
There was no known source of chlorino discharge to DSN 103.
Hundreds of small (ap to six inches long) fish were congreasted in the diffuser pond where the cooler DSN 103 discharge was entering.
This should rule out any suspicion of chlorine as the cause.
They appeared to be trying to 6et into this area of cooler water.
