SUBJECT: NRC INSPECTION REPORT 50-382/94-21 AND MANAGEMENT MEETING
This refers to the inspection conducted by Mr. T. W. Pruett and others of this
office on November 13 through December 24, 1994, and the management meeting
held onsite on December 20, 1994.
The inspection included a review of
activities authorized for your Waterford Steam Electric Station, Unit 3
facility. At the conclusion of the inspection, the findings were discussed
with those members of your staff identified in the enclosed report.
The
management meeting included an update on the implementation of your corrective
action program and the results of your review of operational performance
during recent reactivity events.
The meeting summary is discussed in
Section 10 of the attached report and the meeting slides are provided in
Attachment 3 to the inspection report.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
The results of this inspection are documented in the
enclosed report.
During this inspection period, your staff identified one violation involving
the failure to adequately perform the largest single load reject test of the
emergency diesel generators. However, this violation has not been cited
because the criteria in paragraph VII.B.1 of Appendix C to 10 CFR Part 2 of
the NRC's " Rules of Practice," were satisfied.
You are commended for
identifying and taking proactive and thorough corrective actions for this
violation.
The inspection also identified discernable improvement in the Operations,
Maintenance, and Plant Support areas which indicated that effective corrective
actions have been taken in response to NRC and licensee-identified concerns.
Specifically, operators were responsive to
mormal operating conditions
affecting the feedwater and pressurizer syt
.as and improved log keeping
practices. Maintenance personnel significantly improved cleanliness and
foreign material exclusion controls and showed excellent judgement when tasks
could not be completed as planned.
Additionally, health physics personnel
demonstrated excellent radiological protection oversight skills during the
performance of maintenance and surveillance tests.
The inspector's review of on-line maintenance indicated that scheduling
department practices were effective in providing a conservative approach to
probabilistic risk assessment.
