. In 1996, the U.S. Nuclear Regulatory Commission became aware of specific scenarios where the Amersham Model 660 Posilock radiography camera and the Industrial Nuclear Model IR100 radiography camera would not pass the horizontal shock test as required by 10 CFR Part 34. A retrofit to the lock slide bumpers was issued by Amersham to correct the problem on the Model 660 Posilock camera. A retrofit to the safety latch plate was issued by Industrial Nuclear to correct the problem on the Model IR100 camera. Inspections are necessary to verify the installation and operability of these retrofits. Attached is a draft of a temporary instruction that NRC plans to issue to provide guidance to inspectors. 2800/0qq-02 BACKGROUND in 1996, the U.S. Nuclear Regulatory Commission became aware of specific scenarios where the Amersham model 660 Posilock radiography camera and the Industrial Nuclear model IR100 radiography camera would not pass the horizontal shock test as required by 10 CFR Part 34. A retrofit to the lock slide bumpers was issued by Amersham to correct the problem on the model 660 Posilock camera. A retrofit to the safety latch plate was issued by Industrial Nuclear to correct the problem on the modelIR100 camera. Inspections of the retrofits are necessary to ensure that all cameras have had the retrofit installed.
Appendix A contains Information Notice 96-53, Retrofit to Amersham 660 Posilock Radiography Camera to Correct inconsistency in 10 CFR Part 34 Compatibility, issued October 15,1996, and Supplement 1 to information Notice 96-53, issued June 23,1997, notifying all radiography licensees of the retrofit and informing licensees that effective January 1,1998, units not containing the retrofit will no longer meet the requirements of Part 34.
Appendix B contains Information Notice 97-35, Retrofit to Iredustrial Nuclear IR100 Radiography Camera to Correct inconsistency in 10 CFR Part 34 Compat!bility, issued June 18,1997, notifying all radiography licensees of the retrofit and informing licensees that effective January 1,1998, units not contrining the retrofit will no longer meet the requirements of Part 34.
2800/0qq-03 INSPECTION REQUIREMENTS Each region shall determine the radiography licensees in its region that are licensed to possess the Amersham model 660 Posilock and/or the Industrial Nuclear model IR100. During routine inspections for these licensees, inspectors shall ensure through visual inspection that the required retrofit has been made to all Amersham model 660 Posilock cameras and Industrial Nuclear model IR100 cameras at the inspected facility.

08.02 Resource Estimate. This inspection will be performed during a regularly scheduled inspection. The estimated onsite inspection time necessary to ensure that the retrofit has been installed is minimal (approximately five minutes per unit). The time to compile the data and prepare the report to IMAB/NMSS containing the final statistics is estimated to be less than 16 staff-hours.

Appendix A contains a copy of IN 96-53.
Dmcussion To allow licensees sufficient opportunity to request and install the retrofit, units not containing the retrofit will continue to be approved for use through December 31,1997. Effective January 1,1998, units not containing the retrofit will no longer be approved for use. Affected cameras may be checked for completed retrofit during any inspection. Cameras with the retrofit will be deemed to meet the requirements of 10 CFR Part 34.

Descriotion of Circumstances J
l Recently, NRC became aware of a scenario where the Amersham Model 660 Posilock radiography camera would not pass the horizontal shock test as required by 10 CFR Part 34.20, " Performance Requirements for Radiography Equipment." Amersham was made aware of the test results, and took action to correct the situation.
Discussion When Amersham tested the camera in accordance with ANSI N432-1980 in 1989, the target for the horizontal shock test was a point between the two bumpers, directly at the slide lock. This point was chosen by Amersham as the most vulnerable impact site, in 1995, tests performed as part of an independent testing contract revealed that there was a more vulnerable impact site. The contractor's test focused aiming at one of the bumpers. The result was damage to the bumper, causing the bumper to become detached, thereby exposing the lock slide to direct impact. Subsequent impacts then broke the lock slide. There appear to be no significant safety issues, since breaking the lock slide would result in an inability to open the shutter. If the source were in the exposed position at the time of impact, the source could still be returned to the shielded and secured position.

As a result of the test information, Amersham redesigned the bumpers. The rnodified bumpers contain a metal core that distributes the stress of an impact to the end plate.
Amersham states that the camera with the modified bumpers passed the horizontal shock
. test, where the test was aimed at the bumper, The bumpers remained intact, and although they showed signs of impact, they still protected the lock slide. Amersham states that it sent retrofit kits to all owners, of potentially affected comeras, on record with them. Attachment 1 contains a copy of the retrofit kit letter and bumper replacement instructions. The retrofit will be differentiated by the use of a non Phillips head screw instead of the Phillips head screw currently in use. Cameras with the retrofit would continue to meet the requirements of 10 CFR Part 34.
All affected cameras should be retrofitted as soon as possible. Licensees should contact Amersham to obtain retiofit kits or assistance with retrofit procedures, or to notify Amersham if they have not received their kit. Affected cameras may be checked for completed retrofit during any inspection.
Licensees possessing these cameras should be aware of this issue and take action appropriately.

The corrective action for this is very simple, has been thoroughly tested, and can be easily retrofitted in the field by the customer without any disassembly of the camera. We have f abricated bumpers that will replace the onginal bumpers. These new bumpers have a metal core that distributes the stress of an impact to the end plate. When subjected to the horizontal shock test, the bumpers remain intact, adequately preventing an impact to the lock slide. The retrofst will be differentiated by the use of a non philips head screw instead of the "Philips" head currr ntly in use. We are advising users that upon completion of this upgrade, the onginal certificate we provided stating that the device meets the requirements of 10 CFR Part 34 will still be valid.

Purcose The U.S. Nuclear Regulatory Commission (NRC) is issuing this informatior nive to make licensees aware of a scenario where the Industrial Nuclear Company (INC) ModelIR100 Radiography Camera would not pass the horizontal shock test, as required by 10 CFR Part 34, and of a retrofit to the INC Model IR100 radiography camera, to correct that problem, it is expected that recipients will review the information for applicability to their licensed activities, and consider actions, as appropriate, to avoid problems.
However, suggestions contained in this information notice are not NRC requirements; therefore, no specific action nor written response is required.
Descriotion of Circumstances Recently, NRC became aware of a scenario where the INC ModelIR100 radiography camera would not pass the horizontal shock test, as required by 10 CFR Part 34.20,
" Performance Requirements for Radiography Equipment." INC and the State of California evaluated the problem, and INC took action to correct the situation.
Discussion
!NC recently identified the " Safety Latch Plate" on the IR100 radiography camera as a potentially more " fragile area" (as specified in ANSI N432-1980, Paragraph 8.3.2) than the area previously tested, it appeared that if the Safety Latch Plate were struck directly on the front surf ace of the plate (with the source in the safe locked position), the plate could be deformed and deemed inoperable. It also appeared that the plate might be susceptible to damage if struck from the top and side. To ensure that this type of potential damage was not possible, a bumper that would prevent deformation was welded to the back of the Safety Latch Plate, and the outer top edge of the plate was angled at a 45 to 50 degree angle to a depth of approximately 3 mm (0.12 inch). The modified device was tested to ensure compliance with ANSI N432-1980, paragraph 8.2.
When the source is in the locked, stored position, there appeared to be no significant safety issues, since bending of the Safety Latch Plate prevents source crank out.
However, the State of California has stated that if the source was in the exposed position at the time of impact, the source would Hat be able to be returned to the shielded and secured position.

INC has indicated that it will contact all owners of potentially affected cameras, on record with it, to notify them of the retrofit. INC has recommended that users return the cameras to INC for retrofits. INC has indicated that it can also send retrofit kits and instructions to users that wish to perform their own retrofits. Attachment 1 contains a copy of the retro-fit kit letter and replacement instructions. The retrofit can be differentiated from the original design by verifying that the top part of the plate has a bumper, that is welded on i
the backside of the Safety Latch Plate and which fills a good portion of the space between j
the Safety Latch Plate and the body of the projector.
q To allow licensees sufficient opportunity to request and install the retrofit, units not containing the retrofit will continue to be approved for use through December 31,1997.
1 Effective January 1,1998, units not containing the retrofit will no longer be approved s
for use. Affected cameras may be checked for completed retrofit during any inspectina.
Cameras with the retrofit will be deemed to meet the requirements of 10 CFP % t 04.
All affected cameras should be retrofitted as soon as possible. Licensees should contact 1
INC, or the State of California, to obtain retrofit kits or assistance with retrofit pro-g cedures, or to report that they have not received their kit. Affected cameras may be checked for completed retrofits during any inspection.

(1) With a 5/64 allen wrench remove the 3 set screws of the drive cable receive. Items 8 & 9.

Holding the safety latch plate in the operate position remove the retainer, spring, slide receiver, a Items 11,12,13, & 15.
(B) Remove lock cylinder and safety latch plate.
(1) Witti a 3S2 allen wrench remove the screw under the lock cylinder, hem 10.
(2)
With a long 3/32 allen wrench remove the allen screw in the lock body. Item 3. Note: The screw is inside housing, under the lock cylinder and is accessible only from below. The screw (Item 10) must first be remov (3)
The safety latch plate is spring loaded. Hold down lightly and with the key in the lock slightly rotated, remo eylinder assembly. hem 2.
(4) Remove the safety latch sprPg and pin. Items 4,5, & 6.
RFASSEMBLY OF TifE f OCK (A) Install modified safety latch plate, spring and pin.
(1)
Place exposure device on its side so that the exrosure device handle is on the right.
Install pin, spring and modified safety latch plate in the lock body. Items 4,5, & 6. Note:
plate next to the exposure device body.
Safety latch plate must be installed with the added spacer (B) Install lock cylinder in body.
(1)
Lock cylinder must be installed in the unlocked position with hole in bottom of cylinder down to ahgn with installed through bottom oflock body. Item 2.
(2)
Lock cylinder must be properly inserted in lock body in order for cam to engage safety latch platc. Exc will misalign hole with allen screw and may inhibit safety latch plate action.
(3)
Secure lock cylinder with allen screw. Item 3.
(4)
(C) Install drive cable receiver. Lock cylinder in the unlocked posidon. Test safety latch plate for smooth u (1)
Depress and hold down the safety latch plate (operate position) and insert the slide receiver. Item projecting from the receiver slide must be ahgned so as to enter the small hole inside the lock bod latch plate. Correct rotadon may be verified by observing that the index mark on the face of the receiver is at and that the safety latch plate remains in the operate posidon.
(2)
Insert the large spring in the slide receiver. Item 12.
(3)
Fully insert drive cable receiver /rctainer into lock body cavity and rotate until retainer screw holes align wit sides of the lock body (D) Test for proper asse hem IL Insert allen set screw in bottom of lock body to hold retainct.

Observe that the safety latch plate opening matches the hole through the slide receiver (operate position).
Push drive cable through exposure device and attach dummy pig tail to drive cable.
Pull drive cable with dummy pig tail back (retract) through exposure device. At this point, the safety latch pop up to the latched position. Test for proper action by depressing safety latch plate, push dummy pig tail out an return. With safety latch plate in up posidon and dummy pig tail trapped rotate key to lock posidon. Pull cable rotate key to unlock. Finally, to verify for proper assembly, depression of the safety latch plate to the ope will permit removal of the pig tail through the outlet port of the exposure device.
(2)
Complete insertion of allen screws, Item 8 & 9, described in #7 (above) and lock side screws with special sho Replace screw under the lock cylinder. Item 10.
(E) Test complete system as follows:
(1) Pull back (retract) dummy pig tail.
(2) Safety latch plate will pop up.

Push out (expose). dummy pig tail will not move out of exposure device.
(4)
Rotate key to lock position and remove. Dummy pig tail will not move out of exposure device. Safety la cannot be depressed.
(5)
Insert key and attempt to rotate. (Do Not Force) Key should not rotate to unkrk position.
(6)
Pull back (retract) durnmy pig tail and rotate key to unlock position. Safety latch plate will depress sl 1
