SUBJECT:

ROVIINE RESIDENT INSPECTION AT DAVIS-BESSE (50-346/940ll(DRP))

This refers to the inspection conducted by Messrs. S. Stasek, J. Shine,

D. Kosloff, and D. Shepard of this office on September 14, 1994, through

October 25, 1994.

The inspection included a review of activities authorized

for your Davis-Besse Nuclear Power Station.

At the conclusion of the

inspection, the findings were discussed with those members of your staff

identified in the enclosed report.

Areas examined during the inspection are identified in the report.

Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of

activities in progress.

Based on the results of this inspection, certain of your activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice). The violation is of concern because it involved

weaknesses associated with foreign material exclusion (FME) controls for the

reictor cavity / refuel canal area. Since similar FME weaknesses were

pr(viously identified associated with the spent #

' pool (reference NRC

in;pection report 50-346/94006), we conclude your m lowup actions to that

earlier incident were not sufficiently effective to prevent the subject

violation from occurring.

The NRC is concerned that the potential

,

consequences to plant safety of inadequately implemented FME controls is not

fully recognized by many personnel at Davis-Besse.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

In your

'

response. you should document the specific actions taken and any additional

actions you plan to prevent recurrence. After reviewing your response to this

Notice, including your proposed corrective actions and the results of future

inspections, the NRC will determine whet S r further NRC enforcement action is

necessary to ensure compliance with NRC N.gulatory requirements.
