SUBJECT: Notice of Violation (Inspection Report Nos. 50-373/94017(DRP);

EC-374/94017(DRP))

Dear Mr. Querio:

This refers to the inspection conducted by P. Brochman and other NRC

inspectors, and by R. Zuffa of the Illinois Department of Nuclear Safety, on

August 20 through October 7, 1994. The inspection included a review of

authorized activities for your LaSalle County Station, Units 1 and 2,

facility. At the conclusion of the inspection, the findings were discussed

with those members of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the in pection consisted of selective examinations of procedures

and rerctesentative records, interviews with personnel, and observation of

activities in progress.

During this inspection, certain of your activities related to incorrectly

oriented orifices appeared to be in violation of NRC requirements, as

specified in the enclosed Notice of Violation (Notice). This violation is of

concern because it demonstrates a continued weakness in your corrective action

program. After the close of this inspection, we identified an additional

orifice that was installed backwards. In your response to the Notice of

Violation, please include your plan and schedule for addressing the issue of

'

incorrectl.y oriented orifices in safety-related and important to safety

systems.

Overall, the inspectors determined that engineering performance was acceptable

with some improvements noted. However, the inspectors identified a number of

concerns which merit continued management attention (Section 3 of this report

and Section 3.1 of its attachment). Some of these concerns were identified in

our inspection of progress made in fulfilling your commitments described in

Attachment B to your April 1, 1994, response to inspection reports

50-373/93026; 50-374/93026 and 50-373/93035; 50-374/93035. In addition to

your required response to the Notice of Violation, we request you provide the

projected schedule and current status' of your corrective actions described in

Attachment B to your April 1,1994, response, specifically in the areas of

equipment reliability and preventative maintenance (Sections E.2, F.4.a and

F.4.b).
