j' APPENDIX A NOTICE OF VIOLATION Massachusetts Institute of Technology Docket No. 50-20 Cambridge, Massachusetts As a result of the inspection conducted on December 2-4, 1987, and in accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (Enforcement Policy, 1986), the following violation was identified:

10 CFR 20.203(d) states, in part, that any room, enclosure, or operating area in which airborne radioactive material composed wholly or partly of licensed material exists in concentrations which exceed 25 percent of the amounts specifled in Appendix B, Table I, Column 1 of this part, shall be conspicuously posted with a sign or signs bearing the radiation caution symbol and the words: "Caution: Airborne Radioactivity Area."

Contrary to the above, on December 3, 1987, the airborne concentration of tritium (H-3) within the Equipment Room was in excess of 25 percent of the specified limits in Appendix B, Table I, Column 1, and was not posted with a sign bearing the words "Caution: Airborne Radioactivity Area."

This is a Severity level IV violation (Supplement IV).

Pursuant to the provisions of 10 CFR 2.201, Massachusetts Institute of Technology is hereby required to subniit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Where good cause is shown, considerations will be given to extending this response time.
