Dear Mr. Liu:

SUBJECT: ROUTINE RESIDENT INSPECTION AT DUANE ARNOLD ENERGY CENTER ,

This refers to the inspection conducted by Messrs. J. Hopkins, P. Prescott,

and C. Miller of this office on July 8 through August 23, 1993. The

inspection included a review of activities authorized for your Duane Arnold '

Energy Center (DAEC) facility. At the conclusion of the inspection, the

findings were discussed with those members of your staff identified in the

enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of a selective examination of procedures

and representative records, interviews with personnel, and observation of -

activities in progress.

No violations of NRC requirements were identified during the course of this

inspection.  !

Concerns with foreign material exclusion and personnel errors were identified 1h j

during the inspection. The trip of the spent fuel pool cooling pump due to j'

debris in the suction strainer demonstrates the need for you to improve the

control of- foreign material on the refuel floor. We are also concerned that

the number of personnel errors at the end of the reporting period may be the

beginning of an adverse trend. We will continue to evaluate your efforts to

improve the control of foreign material on the refuel floor and to reduce

personnel errors.

In addition, the results of a routine, announced inspection to evaluate your

10 CFR 50.59 Safety Evaluation Program at DAEC, conducted from June 7 through

June 11, 1993, by Mr. R. Pulsifer of the Office of Nuclear Reactor Regulation,

is included as a separate enclosure (Enclosure 2). No violations of NRC

requirements were identified during Mr. Pulsifer's inspection.
