SUBJECT: NRC INSPECTION REPORT 50-313/97-03; 50-368/97-03 AND NOTICE OF

VIOLATION

Dear Mr. Hutchinson:

An NRC inspection was conducted April 27 through June 7,1997, at your Arkansas

Nuclear One, Units 1 and 2, reactor facility. The enclosed report presents the scope and

results of that inspection.

During the 6-week period covered by this inspection, your conduct of activities at the ANO

facility was generally characterized by safety-conscious operations, sound engineering and

maintenance practices, and appropriate radiological controls. Two violations of NRC

requirements were identified. The first violation resulted from a failure to unisolate an

emergency feedwater pump suction pressure transmitter. Although this violation satisfied

the criteria to be noncited pursuant to Section Vll.B.1 of the NRC's Enforcement Policy, it

is being cited to emphasize the importance of using effective administrative controls to

ensure the proper alignment of risk significant safety systems. The NRC has concluded

that information regarding the reason for the violation and the corrective actions taken and

planned to correct the violation and preverit recurrence is already adequately addressed on

the docket in Licensee Event Report 50-368/97-002-00 dated March 6,1997. Therefore,

you are not required to respond to this letter unless the description herein does not

. accurately reflect your corrective actions or your position, in that case, or if you choose to

provide additional information, you should follow the instructions specified in the enclosed

Notice.

The second violation resulted from the placement of a Unit 2 fuel assembly in a location in

the spent fuel pool which was prohibited by Technical Specifications. As a result of

calculation errors and the failure to perform a required independent verification of the

calculations prior to fuel movement, a total of 14 fuel assemblies would have been placed

in locations prohibited by Technical Specifications had the error not been identified.

Although subsequent analysis demonstrated that fuel in the spent fuel pool would have

remained subcritical, procedures to ensure that fuel placed in the spent fuel pool remains in

a subcritical configuration were not properly utilized by reactor engineers.

circumstances surrounding the violations are described in detail in the enclosed report.

Please note that you are required to respond to this letter and should follow the

instructions specified in the enclosed Notice when preparing your response. The NRC will

use your response, in part, to determine whether further enforcement action is necessary

to ensure compliance with regulatnry requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter,

its enclosure (s), and your response will be placed in the NRC Public Document

Room (PDR). To the extent possible, your response should not include any personal

privacy, proprietary, or safeguards information so that it can be placed in the PDR without

redaction.
