NRC MAINTENANCE INSPECTION REPORT 50-341/97005(DRS) AND

Dear Mr. Gipson:
On May 30,1997, the NRC completed an inspection at your Fermi 2 reactor facility. The
l
purpose of the inspection was to assess the conduct and control of your maintenance
activities and associated engineering support. The enclosed report presents the results of
that inspection.
In general, your maintenance work related to the emergency diesel generator 12 outage
was performed in a well-controlled and efficient manner.
Based on the results of this inspection, the NRC has determined that three violations of
NRC requirements occurred. The violations are cited in the enclosed Notice of Violation
(Notice) and the circumstances surrounding them are described in detailin the subject
inspection report. These violations demonstrate inadequate control of maintenance and
surveillance activities. In particular, your failure to test the diesel driven fire pump to
l
ensure flow and pressure requirements were met following a speed change and your
failure to evaluate surveillance data outside of the acceptable limits is of concern.
Additional examples of violations involving work performed outside the scope of the work
package to repair a spare fuse disconnect switch, and the installation of unqualified parts
on the emergency diesel generators, demonstrated weak centrol of maintenance activities.
We are also concerned that multiple levels of your organization failed to detect,these
problems.
You are required to respond to thic letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part,
to determine whether further enforcement action is necessary to ensure compliance with

regulatory requirements.
