This letter addresses the inspection of your facility at Mount Vernon, Illinois, led by Mr. J. J. Petrosino of this office on November 16-20, 1987, and to the discussions of their findings with Mr. R. Heifner and other members of your l

staff at the conclusion of the inspection.

The purpose of this inspection was to follow-up allegations pertaining to the method in which Nutherm International Incorporated (NI) executes its in-house work activities that affect the quality of comercial nuclear power plant safety-related systems. Areas examined during the inspection and our findings are discussed in the enclosed report.

This inspection consisted of an examin-ation of prccedures and representative records, interviews with personnel, and observations by the inspectors.

During the inspection, it was found that several of the allegation aspects were partially substantiated and that the implementation of your quality assurance (QA) program failed to meet certain NRC requirements. The most significant findings were that electrical devices from two separate purchase orders were observed to be improperly tested, incorrect testing hardware was used, and test activity monitoring to ensure that the devMes were correctly operating for each cycle was not performed. Additionally, two other areas were noted where NI has failed to adequately implement and/or establish its QA program.

It was observed that the majority of NI's technical procedures and instructions had not received an independent review for technical adequacy and examples were identified where the design requirements were not correctly translated into technical testing parameters. Secondly, it was observed that NI has not implemented a QA/QC inspection or monitoring program for its equip-ment testing facility. Therefore, the NRC staff is concerned with the validity of NI's Certificates of Conformance for previously qualified hardware based on the above findings.

It is our understanding that you provided our inspectors with an imediate corrective action proposal, whereby, NI stopped all of its current nuclear related work activities until it could review; revise, if necessary; and reapprove its procedures / instructions; starting with the docu-ments controlling its current in-house orders. Additionally, NI will perform or have a third party perfurm several comprehensive, short duration reviews of its program implementation in certain areas that were discussed during the inspection, but were not reviewed by the NRC inspection team.

Please provide us within 30 days from the date of this letter a written state-ment containing:

(1) a description of steps that have been or will be taken to correct these items; (2) a description of steps that have been or will be take to prevent recurrence; and (3) the dates your corrective actions and preventive measures were or will be completed. Consideration may be given to extending your response time for good cause shown.

The responses requested by this letter are not subject to the clearance proce-dures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.

In accordance with 10 CFR Part 2.790 of the Commission's regulations, a copy of this letter and the enclosed inspection report will be placed in the NRC's Public Document Room.
