(NRC INSPECTION REPORT NOS. 50-327/87-73 AND 50-328/87-73)
This refers to the Nuclear Regulatory Commission (NRC)_ special inspection
conducted by M. W. Branch on October 26-30, 1987, and January 4-14, 1988. The
inspection was conducted to assess TVA's readiness to support Sequoyah Unit 2
entry into Modes 4 and 3.
At the conclusion of the inspection, the findings
were discussed with those members of your staff identified in the enclosed
inspection report.
Areas examined during the inspection are identified in the report, and include:
status of MC 94300 heatup items; testing activities; plant heatup procedures;
TVA's operational readiness assessment; compensatory measures; and control of
'
operations.
Within these a re a s ', the inspection consisted of selective
examinations of procedures and representative records, interviews with
personnel, and observations of activities in progress.
The inspection findings indicate that certain activities appeared to violate
NRC requirements.
The violation, references to pertinent requirements, and
elements to be included in your response are described in the enclosed Notice
of Violation.
This violation involved a failure to adequately perform a
written safety evaluation for modifications to the facility which involved
compensatory actions for defeated safety functions.

This inspection report also identified two issues that should be addressed in

your response: (1) Schedules to complete actions necessary to correct the
defeated safety functions and to eliminate the compensatory measures; and (2)

a determination of the need for an additional assistant shift engineer to be
added to each shift prior to two unit operation.
Four unresolved items were identified concerning:
(1) the need to emphasize
reliance on the containment sump level transmitters; (2) use of administrative
controls to restrict cooling water inlet temperature to safety related coolers;
(3) validation of procedures with compensatory measures, providing additional
details and augmented training as needed; and (4) use of an additional
Assistant Shift Engineer to support two unit operations.
These matters will
be pursued during future inspections.
