Schedule for Reactor Containment Building Integrated Leak Rate Test

Dear Sir:

A Reactor Containment Building integrated leak rate test (Type A test) was conducted at Yankee Nuclear Power Station (YNPS) during the Core XVIII - XIA refueling outage as described in Reference (c).

A leak was discovered at the Fuel Chute Dewatering System blank flange during the initial pressurization. The leak was estimated to exceed the allowed leakage rate and was reduced as much as possible in order to successfully complete the test.

An investigation into the cause for the blank flange leakage is described in LER-87-11 Reference (b).

It was determined that the flange had been improperly installed. The root cause has been attributed to personnel errors and the failure to properly convey design features relative to the installation to maintenance personnel performing the installation.

Yankee in accordance with YNPS Technical Specification 4.6.1.2.b, has scheduled its next Type A test for the Cycle 20-21 outage, summer 1990. This schedule is consistent with requirements in YNPS Technical Specifications and the root cause attributed to the above-described leak.
