Q' HIGHLIGHTS OF WEEKLY INFORMATION REPORT WEEK ENDING MARCH 18, 1988 ALCHEMIE The staff is considering A1ChemIE's request for an exemptica to 10 CFR 550.10(c) to permit the erection of a temporary storage butiding at Oliver Springs Tennessee to house centrifuge machine components to be transferred from the DOE Portsmouth, Ohio facility. AlchemIE needs the temporary storage building to accomodate DOE requirements for the removal of the components from the Portsmouth Gas Centrifuge Enrichment Plant.

Additional information is being requested to substantiate the conditions of the exemption request.

Sequoyah Fuels Corporation, Gore, OK On February 29, Sequoyah Fuels reported from its UFg facility that it possessed an overfilled Model 48G UF tails cylinder (see rep 5rt for week ending March 4,

! 1988). During a site visik to review the matter with Sequoyah fuels, a DOE

! contractor representative from the Paducah Enrichment Plant reconnended that i DOE seek exemption from DOT to permit DOE to ship the cylinder back to the Paducah facility for subsequent handling. An NRC Region IV inspector also was present for the discussions. During the interim, the cylinder remains tagged and stored at the licensee's facility. The DOE investigation into the I circumstances of the overfilling and transfer continues.

Hearinos Two hearings of concern to NRC are scheduled by the Park and Planning Board in May. On April 7, the Board will act on the developers' request to waive the requirement for additional onsite parking until the second building is constructed. On April 14, the Board will meet to reconsider the application to build a second office building on the site, rather than the hotel-conference center it had originally approved.

OFFICE OF NUCLEAR REACTOR REGULATION ITEMS OF INTEREST Week Ending March 18, 1988 Catawba Units 1 and 2 This information supplements the daily highlight of March 10, 1988 which discussed the Unit 2 "A" train AFW pump suction swap-over to the nuclear service water system and the resulting flow degradation.

Because the potential problem could also exist in Unit 1, it was taken to Mode 4 (hot shutdown) at 12:50 p.m. on March 10 after the licensee declared both trains of AFW inoperable and determined that required flow rates could not be assured from the service water system should certain postulated events occur.

Currently, Unit 1 is operating at 15% power. Unit 2 remains in Mode 4 and prior to entering Mode 3 (hot standby) the licensee is comitted to have completed: (a) flushing of the AFW system and reinspection of AFW system flow control valves to the "A" and "B" Steam Generators (CA 56 and 60) for clams when flush is complete and (b) performing a head curve for AFW pump 2A. The licensee has already inspected check valves CA 129 and 3 and found no problems.

Prior to Unit 2 entering Mode 2 (startup), the licensee is committed to have completed an AFW flow balance and a dynamic test (to recreate the Unit 2 event of March 9, 1988) while monitoring the suction pressure. The licensee will compare the measured suction pressures to calculated values.

The AIT is monitoring the licensee's activities and consulting with NRR and Region 11 regarding the restart of Unit 2.

This report describes the March 9, 1988 unit scram. While performing the functional test on a differential pressure switch, an Instrument Maintenance l (IM) technician inadvertently valved in the variable and reference legs of the switch with the equalizing valve open. This initiated a "pressure equalization" l between the variable and reference legs, and resulted in a high "indicated" level to feedwater level control, causing the feedwater pumps to begin reducing low. Realizing the valving error was made, the reference leg was immediately isolated from the variable leg which resulted in a low "indicated" level spike.

The level spike caused other level switches, utilizing the same reference leg, to also actuate, including the trip of the Reactor Recirculation (RR) pumps frcm i an Anticipated Transient Without Scram (ATWS) signal.

OFFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending March 18, 1988 LOW-LEVEL WASTE MANAGEMENT Low-Level Wasle Hydrogeologic Aspects of Rad _ioactive Waste Disposa_1 On March 12, 1988, LLWM staff convened a techni:al symposierr entitled, "Hydrogeologie Aspects of Radioactive Waste Disposal" at the 23rd Annual Meeting of Northeastern Section of the Geological Society of America in Portland, Maine. The symposium provided a forum for presentation of hydrogeologic assessments of existing radioactive waste disposal sites in the United States, and for discussion of current topics such as the effects of spatial variability of hydraulic properties on radionuclide transport.

Several NMSS and RES staff cnd contractors participated in the symposium in addition to representatives from the U.S. Geological Survey and contractors for the U.S. Department of Energy.

0A Training The Division of Low Level Waste Management and Decommissioning is conducting a two-day, in-house training course on the application of Quality Assurance (QA) for a low-level radioactive waste disposal facility on March 21 and 22 at White Flint North.

FRN on Completion of Section 8(b) of Low-Level Radioactive Waste Policy Amendments Act of 1985 (LLRWPAA)

The Division of Low-Level Waste Management and Decommissioning has issued NUREG-1199, Rev.1. "Standard Format and Content of a License Application for a low-Level Radioactive Waste Disposal Facility" and NUREG-1200, Rev.1, "Standard Review Plan for the Review of a License Application for a Low-Level Radioactive Waste Disposal Facility." These NUREG reports provide timely and complete guidance on alternative disposal options. In addition, these NUREGs incorporate major revisions in the areas of financial assurance and quality assurance. The issuing of these NUREGs fulfills NRC requirements under Section 8(b) of LLRWPAA.

2 SAFEGUARDS International Implementation of the US/IAEA Aareement On March 15, 1988, the Safeguards Licensing Branch issued license conditions to incorporate the IAEA Facility Attachment to the General Electric low enriched fuel fabrication facility at Wil.nington, North Carolina. This action implements full IAEA safeguards, including inspection, at the facility under the provisions of the US/IAEA Safeguards Agreement.

Transportation Plutoniun Air Transport On March 14, 1988, SGTR staff met with Japanese experts and officials to discuss the safe transport of plutonium by air. Japan has an effort underway to formulate national safety standards for air transport of plutonium. This effort has been in process for about 3 years. Views were exchanged on the technical considerations for safe transport including the severity of aircraft accidents, conservatisms of tests conducted on essentially unyielding targets, contribution to safety of package tie-downs in severe accidents, burial of packages and the impact tests in NUREG-0360 with respect to worse case accidents. The staff experiences concerning the development of "Qualification Criteria to Certify a Package for Air Transport of Plutonium" (NUREG-0360) and the certification of Model Nos. PAT-1 and PAT-2 were also discussed.

The Japanese officials were interested in the implementation of the recent U.S. law (Murkowski amendments) which places additional requirements on the shipment of plutonium by air, through the air space of the U.S., from one foreign country to another. It was indicated that a letter of intent to conduct such shipments may be sent to the NRC.

The Japanese officials expressed a desire to have an additional meeting in June / July to discuss plutonium air transport.

Cask Trailer Roll-Over On March 11, 1988, a trailer overturned while carrying an NRC certified spent fuel cask. The cask was empty at the time of the accident and no radioactive material was released. The accident occurred on-site at a DOE facility (Rockwell International). The truck was making a 90* turn at about 10 mph when the accident occurred. DOE is investigating the accident.

e 0FFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending March 18, 1988 PREVENTING DAMAGE TO THE REACTOR CORES Technical Support Center (TSC)

The TSC has provided draft input to the NRC for development of an NRC severe accident management plan. This input, provided on March 16, 1988, addresses in-vessel accident management issues, specifically (1) primary system depres-surization, and (2) secondary system depressurization, shutdown, information available to the operators, operating procedures and uncertainties. The work has been coordinated with staff from Battelle Columbus Laboratories who have responsibility for (1) water addition to the primary system, (2) water addition to the secondary system, and (3) infomation available to the operators. The results for individual tasks were reviewed by each laboratory and with the NRC staff.

One of the principal tasks of TSC is to provide short turnaround research applicable to current priority issues. A priority study was initiated this week to provide scoping infomation for transients initiated by reactor trips with failure of turbine trip. One possible consequence of this transient is severe pressurized themal shock since core power is decreasing to decay levels while heat transfer to the secondary coolant is rapidly increased as steam flow to the turbine increases. The scoping study will provide the staff with information needed to assign a priority to the generic issue. Analyses of plant responses of a B&W plant and a CE plant to failure of the turbine trip from a range of powers will be evaluated. Among these enalyses will be one which simulates the recent transient at the Crystal River plant in which reactor trip occurred but turbine trip did not.

RADIATION PROTECTION & HEALTH EFFECTS Safety Requirements for Industrial Radiography Equipment Following unanimous approval by the Commission, a proposed rule which would amend the regulations dealing with industrial radiographic equipment to establish new regulate y requirements in the form of perfomance standards for such equip' .nt was published for coment in the Federal Register on March 15, 1988 (53 FR 8460). Coment period for the proposed rule expires on May 16, 1988. The performance standards proposed consist of incorporating by reference the American National Standard N432, "Radiological Safety for the Design and Construction of Apparatus for MARCH 18, 1988 ENCLOSURE D L

2 Gamma Radiography," plus a number of additional performance standards recomended by the Radiography Equipment Task Force created by the ad hoc Radiography Steering Comittee founded in 1980. In addition to the performance standards the proposed rule includes a requirement for radiographers to wear alarm dosimeters. The purpose of the proposed rule is to attempt to reduce the number of overexposures to both radiographers and the general public from the use of this equipment and the high intensity gamma-ray sources that are contained in the equipment.

This regulatory guide describes a method acceptable to the NRC staff for complying with NRC's regulations on seismic qualification of electrical and nechanical equipment.

. Items of Interest Office for Analysis and Evaluation of Operational Data Week Ending March 18, 1988 Incident Response On March 15, the Director, AE0D, and members of the Incident Response Branch (IRB) and Congressional Affairs staff met with Congressmen Huckaby and Gejdenson and their staffs to discuss the issues related to activation of the Energency Response Data System (ERDS). The issues were discussed in connection with H.R. 1570, the ERDS Act of 1988. It appears that the Congressmen have agreed upon requiring licensee transmission of data at the Unusual Event level with provisions for also obtaining earlier data. This is in lieu of requiring automatice transmission of data when certain conditions were reached.

Congressman Gejdenson indicated an interest in visiting the Operations Center.

During the week of March 14-18, IRB staff members conducted an assessment of the emergency response program in Region I. The principal areas reviewed were the region's initial response to events reported by licensees and the standardization of procedures, training and equipment for regional response to events.

On Tuesday Mr. Takao Numakunai, Director, Department of Health Physics, Japan Atomic Energy Research Institute, accompanied by Mr. Yutaka Kawakami and Mr.

US Ecology's decision was delayed pending receipt of guidance from the California Department of Health Services (DHS) regarding desert tortoise concerns. DHS. the State Agreement agency responsible for coordinating environmental reviews and licensing the project, inforned US Ecology that potential adverse impacts on the desert tortoise in the relatively small area involved should not override public health and safety factors. The Departnent concurred in US Ecology's conclusion that the Ward Valley site is technically superior to other sites examined, and expressed plans to identify and require mitigation and compensation measures necessary to protect the desert tortoise.

In-depth investigation of the Ward Valley site will now intensify to support preparation of a Proponents Environmental Assessment and a License Application.

A Local Citizens Committee established for that site will be invited to continue working wth US Ecology to help ensure that local concerns are addressed in the developnent proposal. US Ecology anticipates applying for a license about a year from now with the intent to construct the site and begin accepting waste in 1990.

Uranium Mill Tailings Meeting On March 17, 1988, State, Local and Indian Tribe Programs (SLITP) held a meeting in Region IV's offices with certain Agreement States and NRC Staff to discuss the technical, legal, and policy issues involving the disposal of various categories of materials associated with uranium processing activities in mill tailings piles. The issues were raised by Texas and the Uranium Recovery field Office as natters needing resolution. Participants it cluded MARCH 18, 1988 ENCLOSURE F

representatives from SLITP; the Office of the General Counsel; the Office of Nuclear Material Safety and Safeguards; Region IV; and the States of Texas, Utah, Washington, and Colorado.

Lecture Before Dupont Company Radiation Safety Officers On March 16, 1988, Kathleen N. Schneider, SLITP, participated in the Dupont course on Fundamental Management and Control of Radioactive Material and Radiation for Dupont Radiation Safety Officers in Billerica, Massachusetts.

She presented a lecture on the regulatory program of NRC including discussion on NRC regulations and a lecture on the Agreement State program.

ACRS Subcomittee on Waste Management Briefing on LLW Disposal in the States On March 17, 1988 Steve Salomon, SLITP, briefed the ACRS Subcomittee on Waste Management in Washington, D.C. on the current status of States in providing disposal of low-level radioactive waste. Issues addressed included the January 1,1988 Congressional milestone for designation of host States for compacts and siting plans, the proliferation of sites, costs of disposal, emerging preferences in alternative disposal technologies, and additional compact efforts.

OFFICE OF PERSONNEL ITEMS OF INTEREST Week Ending March 18, 1988 Recruitrent Frank Elizondo, OP's Recruitment Officer, accompanied by representatives from NRR, NMSS, and Region I recruited at the Massachusetts Institute of Technology on March 14, 1988. Personnel from OP, OSDBU/CR and Region 11 attended the National Conference of the Society of Black Engineers in Washington, D.C., on March 18, 1988. As a result of this conference, Mr. Elizondo invited five female engineering students and one faculty member to One White Flint North for discussions with NRR and NMSS staff atout their organizational responsibilities and employnent oppo ',snities (entry-level, co-op, summer faculty).

Training Data Base OP in cooperation with ARM is in the process of implementing a relational data base to provide informatirl on necessary, existing and future training for NRC employees Agency-wide. Tais data base will initially serve the regions and program offices. NRR is the initial requesting office.

0FFICE OF ENFORCEMENT ltems of Interest Week Ending March 18, 1988 The following Sionificant Enforcement Actions were taken the week of March 14, 1988.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $100,000 was issued on March 14, 1988 to Niagara Mohawk Power Corporation (Nine Mile Point Unit 1). This action was based on operation of the plant between June 1986 and September 1987 without dispositioning certain weld flaw indications which were identified by the inservice inspections conducted during the 1986 refueling cutage. The civil penalty was increased 100% because of the licensee's prior poor performance in the area of concern. (EN 88-19)

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $100,000 was issued on March 14, 1988 to Arkansas Power and Light Company (Arkansas Nuclear One Unit 1). This action was based on a failure to take corrective action for a long standing problem of containment building temperatures in excess of those assumed in the plant design basis. The civil penalty was increased 1007 because of the duration of the violation. (EN 88-20)

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $25,000 was issued on March 14, 1988 to Nebraska Public Power District (Cooper Nuclear Station). This action was based on an inadequate search of a contractor's vehicle that resulted in the introduction of ammunition to the protected area. The civil penalty was reduced by 50% because of the licensee's identification and reporting of the incident. (EN 88-21)

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $100,000 was issued on March 17, 1988 to Florida Power Corporation (Crystal River Facility). The action was based on violations involving the failure to:

(1) lock, post, barricade, and issue a Radiation Work Permit to control access to a high radiation area; (2) provide adequate instruction and training to l

employees working in a restricted area; (3) establish adequate procedures j for radiation protection and refueling operations; (4) provide a radiation

- nonitoring device to an individual inside a posted High Radiation Area as

! required by your Technical Specifications; and (5) follow radiation control

! procedures. The civil penalty was increased 100% because the licensee had l prior notice of similar events and some of the violations are similar to violations occurring in 1986. (EN 88-23)

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $100,000 was issued on March 17, 1988 to Wolf Creek Nuclear Operating Corporation (Wolf Creek Generating Station). The action was based on l

several examples of significant procedural control weaknesses during the performance of outage activities. These weaknesses resulted in, among other things, improper maintenance on safety-related switchgear, contamination of personnel, release of potentially contaminated materials to a landfill, ignition of hydrogen in the pressurizer, an engineered safeguards actuation causing lake water to enter the steam generators, and the movement of fuel l

when required monitors were inoperable. The civil penalty was increased 100% because of the number of violations and the failure to taken prompt corrective actions. (EN 88-22)

A Confirmatory Order Modifying License (Effective Imediately) was issued to Georgia Institute of Technology, Atlanta, Georgia on March 17, 1988. The Order confinned the licensee's commitment to continue the suspension of all reactor operations until the licensee has resolved all-safety questions to its satisfaction and the NRC has approved in writing, the resumption of operations. (EN 88-02A)

Civil Penalties Paid Northern States Power Company (Monticello Nuclear Generating Plant) paid the imposed Order in the arrcunt of $50,000. The action was based on the failure to establish and implement a procedure to evaluate the effects of electrical design changes on other portions of the systen in the area of electrical coordination. This led to a change being made which created an unreviewed safety question in that faults on non safety-related components could cause the loss of multiple essential buses during a design basis event. (EA 87-147)

The Detroit Edison Company (Fermi, Unit 2) paid the proposed civil penalty in the amount of $25,000. The action was based on a design error discovered in the circuitry of the swing electrical bus which would have resulted in the loss of both divisions of low pressure ecolant injection during an accident condition. (EA 87-232)

OFFICE OF CONSOLIDATION Items of Interest Week Ending March 18, 1988 Parking The developers of One White Flint North have decided to. discontinue use of the "satellite" lot at White Flint Mall for NRC employee parking. The Montgomery County Planning Board had cited the firm for violating a zoning ordinance by providing supplemental parking at the mall rather than onsite, and an appli-cation for a zoning exception would have been necessary to continue its use.

Since only six permits for parking at that lot were sold in March, the application will not be filed.

As construction material is being cleared out of the parking levels in the building, approximately 70 additional spaces will become available in April in the garage. With the coming of warm weather, it is anticipated that more people will park at the County "Park and Ride" lot, and that most of the demand for indoor parking can be met.

The developers plan to provide sufficient parking spaces at the second building to meet the county's requiromants for both One and Two White Flint North.

Hearinas Two hearings of concern to NRC are scheduled by the Park and Planning Board in May. On April 7, the Board will act on the developers' request to waive the requirement for additional onsite parking until the second building is constructed. On April 14, the Board will meet to reconsider the application to build a second office building on the site, rather than the hotel-conference center it had origir. ally approved, i

REGION II ITEMS OF INTEREST WEEK ENDING MARCH 18, 1988

1. On March 15 and 16, the Regional Administrator was in Headquarters to participate in the ED0's Senior Management Meeting.
2. On March 16, Chairman Zech, accompanied by his Technical Assistant, the Project Manager for Surry, and selected Region II staff members, was at the Virginia Electric and Power Ccmpany's Surry facility for site familiarization.
3. On March 16, representatives of the Duke Power Company were in the Region II Office to attend a meeting regarding issues surrounding the emergency power switching logic at their Oconee Nuclear Power Station.
4. On March 18, the Deputy Regional Administrator, accompanied by Branch Chiefs from the Division of Reactor Projects and the Division of Reactor Safety, participated in the PRA Team Inspection Exit Meeting at the Carolina Power & Light Company's Brunswick site. The principal purpose of the inspection was to assess the capability of the plant, personnel, and procedures to respond to degraded core accident sequences. The Brunswick containment is a Mark I design, and the inspection included an assessment of the capability to vent containment.
5. A confirmetory order was issued to Georgia Tech on March 17 confirming that their reactor would remain shut down until Georgia Tech submits their justification for startup and the NRC agrees in writing. Subsequent to the January 20 NRC order to cease irradiation experirents, the Pre ident of Georgia Tech shut the reactor down completely on February 15 pending the perf9 mance of an inde-pendent evaluation of Georgia Tech's progr . and procedures.

1. An enforcement conference was held on March 15, 1988 with Omaha Public Power District management concerning missing electrical conduit fire barriers.
2. A meeting was held on March 17 in the Region IV offices regarding a request from Texas for clarification of issues and definitions of byproduct material, low level radioactive waste, rare earth facility waste, etc. The meeting was coordinated by State, local and Indian Tribe Programs, GPA. Representatives from four states attended the meeting (Texas, Colorado, Washington, and Utah).
3. A meeting was held in the URF0 Conference Room on March 17 with Western Nuclear, Inc., on the proposed enhanced evaporation system for the Split Rock Mill.
4. Representatives from the NRC's Uranium Recovery Field Office met with American Nuclear Corporation in Denver, Colorado, on March 18, to discuss the licensee's proposed Mill Dec'smmissioning Plan.
