NRC INTEGRATED INSPECTION REPORT 50-413/97-07 AND 50-414/97-07 AND

NOTICE OF VIOLATION

Dear Mr. McCollum:

On April 26, 1997, the NRC completed an inspection at your Catawba Nuclear

Station.

This inspection period covered most of the Unit 2 End-of-Cycle 9

l

Refueling Outage. This report presents the results of that inspection.

During this inspection period. violations of NRC requirements were identified

and assessed as having more than minor safety significance.

These violations

,

are cited in the enclosed Notice of Violation (Notice).

The first involved a

'

l

Unit 2 loss of spent fuel pool cooling caused by an inadequate procedure. The

second involved untimely actions to correct a deficiency associated with the

i

fire pump intake screens..

'

Operator performance during the outage was generally good; however, the loss

i

of spent fuel pool cooling could have been prevented with more diligent

control board monitoring and a more complete pre-job briefing.

Maintenance

i

,

activities reviewed during the outage were performed appropriately, and

i

maintenance self-assessments have contributed to improved performance.

'

Engineering activities that were reviewed demonstrated appropriate design

,

control for modifications and good identification and resolution of an

l

i

equipment problem via the Operating Experience Program.

Radiation protection

activities were well controlled, and aside from the cited violation, the

overall assessment of fire protection programs was good.

.

You are required to respond to this-letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

The NRC will

'

use your response, in part, to determine if further enforcement action is

necessary to ensure compliance with regulatory requirements.
