Gentlemen: l On October 2,1975, the Commission issued technical specifications that revised the reporting requirements for the Yankee-Rowe Atomic Power Station. . In the letter, we requested that you use the format of the Appendices to Regulatory Guide 1.16, Revision 4. Instructions for using the reporting formats were contained in AEC report 00E-SS-001, "Instruc-tions for Preparation of Data Entry Sheetr. for I.icensee Event Report (LER)

File", c::4 updated instructions dated August 21, 1975. The purpose of this letter is to replace the first page of the updated instructions dated August 21, 1975, with the enclosed sheet dated December 8, 1975.

A typographical error was corrected and the location of the correction is indicated by a mark in the margin of the December 8,1975 sheet.

.9 December 8, 1975 Operating Nuclear Power Plant Licensees INSTRUCTIONS FOR PREPARATION OF DATA ENTRY SHEETS.FOR LICENSEE  !

CVENTREPORT(LER) FILE The Energy Reorganization Act 'of 1974 Section 208, defines.

" abnormal occurrence" as an unscheduled incident or event the

  • Comission determines is significant from the standpoint of l public health or safety. Accordingly the term " abnormal occurrence" is reserved for usage by NRC. This restricted  ;

definition and other requirements for Section 208 will require ,

changes in the subject instructions, but for the immediate future the existing instructions can be utilized, as modified by this letter. -

The original instructions were issued in October 1974 as 00E-SS-001. The enclosure, an update to the instructions, was  !

sent to the licensees after the November 1974 regional meetings i which included discussions on the instructions. The following changes are hereby made to be effective as of receipt.

1. The term " abnormal occurrence" is to be used only by NRC in its reports to the Congress and the public.

All reports from licensees should be termed " reportable occurrences".

Licensee Event Report is an acceptable generic term. Any reference _ to, or use of the terms abnormal occurrence or A0 should cease in licensee event reporting. l

. Item 5 -- on the LER form (Event type in 00E-SS-001) should  ;

, be interpreted as follows.

01 - is to be used when referring to events requiring prompt notification (Regulatory Guide 1.16, Rev. 4, C.2.a).

5 03 - is to be used when referring to events requiring 30 day '

notification (Regulatory Guide 1.16, Rev. 4, C.2.b).

2. All classificatie'n of report significance is to be done by NRC. f Categorization by the licensee as " Prompt Notification" and 3

" Thirty-Day" reports will suffice as an indication of potential significance.

Item 6 -- on the LER form (Category in 00E-SS-001) is not to be utilized. Correction nos. 5, 6, and 7 on the enclosure are to be deleted and are so marked. All of item 6 in 00E-SS-001, pages 11, 12, 13, 14, 15 and the top of page 16 should be deleted also. i l

3. Special effort should be made to include the essential information required by Section 208, in case the event is identified by NRC as an abnormal occurrence. This means including:

(1) Date and place (2) Name and probable consequences (3)Causeorcauses,and (4) Any action taken to prevent recurrence
