CONFIRMATION OF MEETING - SEQUOYAH DOCKET NOS. 50-327 AND 50-328 This confirms the telephone conversation between Mr. S. A. White, Manager of Nuclear Power - TVA, and Mr. S. D. Ebneter, Director, Office of Special Projects - NRC, on March 14, 1988, concerning an Enforcement Conference to be conducted at the Sequoyah Nuclear Power Plant, Soddy Daisy, TN on March 17, 1988, at 1:00 p.m. The purpose of this meeting is to discuss concerns related to the apparent failure to comply with Technical Specification (TS) 3.0.3 action requirements when both centrifugal charging pumps were IN0PERABLE for a 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> and 24 minute period on March 9,1988. In addition to this specific event, we request that you be prepared to address why the corrective action your staff has taken for other recent operational events was not effective in preventing this occurrence. A proposed meeting agenda which lists our major  ;

concerns is enclosed.

Why the event occurred Why it went unnoticed during shift turnover Why did the operators on shift at'the time of discovery consider CCP 2A OPERABLE while it was in "Pull-to-Lock" Why it took so long to notify the shif t supervisor Why it took so long for the event to be reported to NRC Headquarters (Red Phone)

(2) Why corrective action for those recent operational events listed below did not prevent the March 9, 1988, occurrence:
